posting
this
on
8/1/23,
I
have
been
trying
to
access
my
medical
records
so
that
I
may
enlist
in
the
Army
since
7/12/23.
As
a
military
brat,
I
have
been
seen
at
several
clinics
in
several
states
throughout
my
life,
and
Elms
Creek
Clinic
is
the
only
one
that
has
refused
to
be
helpful
in
my
attempt
to
obtain
my
medical
records.
I
have
called
a
total
of
13
times
since
7/12/23
and
almost
every
time
been
told
I
must
come
in
person
to
gain
access.
I
live
in
Kentucky,
I
do
not
have
time
nor
the
financial
ability
to
travel
all
the
way
back
to
Ft.
Hood/Killeen
to
obtain
medical
records.
Every
other
facility
I
have
had
to
obtain
medical
records
from
during
my
recruiting
process
has
emailed
me
a
medical
release
form
and
instructed
me
to
email
it
back
to
them
with
a
copy
of
my
Identification
attached.
Each
other
facility
has
been
able
to
work
with
me
and
send
my
records
over
within
1
business
day.
Elms
creek
has
not
even
attempted
to
provide
me
with
a
medical
release
form
or
anything
of
the
sort
that
would
grant
me
access
to
my
medical
records.
The
office
manager
actually
threatened
to
withhold
them
from
me
in
my
most
recent
correspondence
with
her.
This
is
the
only
piece
of
paperwork
I
need
so
that
I
may
complete
my
enlistment
process,
and
they
are
single-handedly
preventing
that
from
happening
through
their
negligence.
I
am
contacting
the
Texas
Medical
Board
about
my
situation.
P.S.-I
had
nothing
but
positive
experiences
with
Dr.
Ricky
Mitchell
throughout
my
time
as
a
patient
at
Elms
Creek
Clinic,
I
wish
their
customer
service
staff
lived
up
to
the
high
performance
that
Dr.
Mitchell
exhibits
day
in
and
day
out.
