    I wish I could give this place zero stars. This is the perfect example of beautiful on the outside and ugly on the inside. I picked this place for my father to have rehab after his stroke. He arrived very conscious and alert, after being at The Rivers for less than 2 weeks we noticed a change in his alertness. We spoke with his nurses everyday and even asked when will the doctor come in to see him. They responded by saying the doctors come in at 5:00 am in the morning , visiting hours did not start till 8:00 am how was I suppose to ever speak to them. After being taken out to dialysis the receptionist at the dialysis center sent my father to the hospital. He had a bed sore that had gotten to STAGE 4. My question is HOW COULD YOU MISS THAT? Where were the 5:00 am "doctors" where were the "Registered Nurses"? After several days in the hospital and a very painful surgery to remove the infection and skin graphs my dad was discharged and the hospital stated they will not discharge him back to this facility. I went to get my father things and was told I need to speak with the Director of nurses but she was in a meeting, which was a big laugh because the social worker at the hospital told me she has tried to contact the director of nurses on several occasions and does not believe she exist. So I left my information for her to call me and I still haven't received a call back. The day I went to get my father things everyone that worked there was in the patients lounge laughing and talking they didn't even notice me pack up his things and leave. As I was walking out I noticed a man leaning over the side of his bed with his nurse light on and I asked him did he need me to get someone for him and he said yes. I went and told a nurse and waited for her to go see about him which was 15 minutes later, because I figured out that is what you have to do around there. I'm not sure who runs the place but no one there cares about whats going on. Every time I would call Or visit I would hear the bells ringing from the call lights that will never get answered. I hope this review stops someone from making the worst mistake of their life. On the bright side of this if your family member is self sufficient and can walk and talk and do everything for themselves then this place is like a really cute apartment, but if not stay far, far away.

    Don't. Even. Think. About. It. You cannot even imagine the incompetence. When we arrived, the sheet wasn't even pulled out around the edges of the bed. No one came to the room to welcome the patient, go through his medications, give him an idea of when the doctor would be visiting, nothing. When you ask for a glass of water, it takes an hour. There is no sense of structure or organization at this place. The food is terrible. One day a plumber comes in to fix a toilet that during the entire duration of the stay was never working and when the plumber saw a pack of wipes which were not being used for the toilet but for another purpose, he admonished the patient and told him he could not have them in his room. When the patient agreed and asked to have them left in his top drawer, the plumber said that he could retrieve the wipes from the front desk. Well, you can imagine how well-tracked a package of wipes was at this place. We never saw them again. If you want your loved one to be disciplined by a plumber, send him/her to this place. If not, then skip this place altogether. It is truly the worst.

    if your loved one is here TAKE THEM OUT IMMEDIATELY. This place is elder abuse, no doubt about it There are three parts to The Rivers: Skilled Nursing/Rehab, Assisted Living/Memory Care, and Independent Living. I worked Memory Care. I left because of the risks this facility puts on my license and, more importantly, how much it broke my heart to see the deplorable care these residents receive. Memory Care is considered Assisted Living, so it isn't covered under OBRA. Long Term Care facilities like the Skilled Nursing side have LPNs and CNAs to take care of their residents; legally, they can't hire someone who isn't licensed in either category. Assisted Living doesn't have that regulation LEGALLY, although most hire CNAs regardless. Instead they hire RCAs, who don't have any license and have no health care training. They're trained "on the job," which is a joke at this facility. During my employment I witnessed more elder abuse that I have ever encountered. Residents being manhandled, yelled at, neglected, and verbally abused. The Memory Care unit requires NO dementia training. I was given to training in dementia, and had none previously. These RCAs and Med Techs (NOT nurses: there is no nurse on the memory care floor) are not trained in patient care, let alone things like Alzheimer's. They blame the residents for their behaviors and get agitated and lash out at them. The floor doesn't have any stimulation to redirect them with; most facilities have something for the residents to distract themselves with, like dominoes or felt and Velcro. It keeps their mind active and helps redirect them when they're agitated; this facility had nothing of the sort. They have a TV. That's it. Activities will come and do something with the residents every day, and that department (from all i saw) was actually not bad; however, they're only there a few hours a day. That's the only social contact or stimulation these poor residents get. On my first day I arrived at the facility at 7am for my shift; there were no RCAs or Med Techs in site. The floor was empty. ALL health care facilities have handover between shifts, which allows oncoming staff to review how the patients/residents are doing with the previous shift. Doctor's, nurses, CNAs, any patient care providers you can think of. I have never seen a health care facility that didn't do handover, it's extremely unsafe. How can 3rd leave when 1st shift hadn't arrived? What if something happened while no one was on the floor? I have no idea how long these residents were left unattended. People leave around when their shift is over and come in around when their shift is starting. That same day we didn't have a med tech for the first hour of the shift; there was one CNA (me) and one RCA for an entire hour. No one able to or having authorization to open the med cart. I could go on (for pages and pages) but I'm running out of characters. I also found it strange the fact that two non nursing employees felt the need to point out how I was "always working" ---implying the alternative is the norm at this facility. Which, it is. I'm counting the days until this facility is shut down. Shame on the administrators who work here and allow this to happen. This is abuse.

    The Rivers of Grosse Pointe is separated into 3 facilities under 1 roof: Independent Living, Assisted Living/Memory Care and finally a Rehab center. My grandma stayed at all 3 areas over the last 3 years so I feel confident that I can give you a comprehensive review of all facets of the Rivers. First the good. Walking into the independent living facility is like walking into a new hotel. It is light, bright, has high end finishes and furnishings, and the friendly staff at the front desk are always helpful. The apartments have tall ceilings, high end finishes and are quite spacious. They are able to get a nice cable TV package. It is a very nice place to live as long as your loved one will never need any further care. While living in the independent living apartment, my grandma fell getting out of bed. There is no way to contact the front desk or anyone from the floor of her apartment. It was hours before the neighbors below her heard her cries and tried to find anyone working at the 5 am hour. They found a cafeteria worker coming in for the day who helped call the ambulance. After her stint in the hospital, my grandma was sent to the rehab unit. This is very similar to a hospital ward except it has about 1/10 of the staff. It looks like a nice place, especially since there is a huge Physical Therapy Gym attached to the facility. You can hardly find anyone working there and when you do, they are clearly overworked and overwhelmed. My grandma doesn’t like to be a bother and was largely ignored, even taking a fall while there as she attempted to use the bathroom. I do not recommend coming here to rehab. It was determined that my grandma needed some assistance (getting out of bed, other tasks that could endanger another fall) so Rivers has NO option for that in the independent living side. You have to move everything in their apartment to the assisted living side largely on your own. When you walk into the assisted living side, you need to sign in with an security guard. You need to remember to take a key card as the residents are locked in their own hallway. The staff is also overworked but less so than the rehab side. The rooms at assisted living range from a double hospital room, single hospital room and a deluxe suite which has an area for sleeping a little further away from a living area. But with no kitchen, no storage and a view that consisted of a swampy lawn backed up by a parking lot, it is not a nice place to live. No cable package is available and the TV is analog. No options to upgrade and you can’t get your own cable system. Why does the administration feel that the assisted living people don’t deserve decent TV? Food is included, if you like what they are serving that day, and you enjoy sitting in the eating area next to the nurses station with the continual beeping of the unanswered call buttons. Finally, after living in the assisted living facility for 6 months, the administration increased my grandma’s monthly rent by 63%! Preying on the elderly is a shameful practice. Everyone gets older. Think about that before deciding to live at Rivers as you may have to move your family multiple times as he/she gets older. Do yourself a favor and find an Independent Living facility that has onsite help that can be added on as your family member needs future assistance.

    If I could, I would give them negative stars. The facility looked nice however the treatment was far from it. My mother went there on June 23 and my sister had to remove her on July 5. My mom went there for rehab to help her with her walking. While there she sustained a couple of small cuts/scrapes while the staff were moving her. The minor cuts were not properly treated and when my sister went to see her, her lower leg was the size of a person's thigh. She had been sitting in a chair for 8 hours a day, without treatment or even having her leg elevated. My sister had taken bottled water for her to drink since she did not like the taste of tap water and all the bottles were still there. The staff were not giving her the water and she was dehydrated. My sister took her home. The untreated wound turned into an infection and my mother had to be admitted to St. John's Hospital where she remains today in intensive care. While at The Rivers she did to received any antibiotics. She is an insulin dependent diabetic and we are praying that this does not end in an amputation. My mom said a nurse told her that they were understaffed and it was 30 patients to one nurse. If the facility cannot handle the patients due to a staff shortage they should not continue to accept new patients. I will be contacting BC/BS, medicare and finding out who else I can report this facility to for their neglect. Do not take a loved one to this facility. For the fool that said if you don't like it you can care for the person at home.....the point of the person being there is for CARE that they cannot receive at home. This is not a FREE service. This facility it being paid to do a job and it should not be out of the realm of possibility to expect that they will fulfill their obligation to care properly for a patient. If they can't do it, close up. All these people "complaining" simply expected their loved one to get the care that is promised. If not, they would have taken the person somewhere else to begin with. What an ignorant statement.

    My mother was at the Rivers physical rehab. I would NOT recommend this facility from the nursing standpoint. Due to Covid precautions family could not go in facility. Attempts to communicate with nursing staff and Vermon, the nursing administrator, we were largely ignored and calls were not returned. The did not communicate with my mother’s dialysis facility and did not complete necessary daily paperwork for dialysis facility to accurately assess how her renal status. I do not believe they accurately completed the peritoneal dialysis nightly due to many issues. Ultimately they sent my mom on their regular transport van to an appointment with a blood pressure of 70/40 (very low). When she arrived at her appt she was immediately sent by the physician to the ER by ambulance. My mother had emergency surgery & died. No one in the facility would talk to family despite repeated calls to the nursing director of the rehab and director the Rivers facility. A complaint was filed by the family & after the state investigated, they were found to be cited in violation and needed to complete the corrections. In the records that we requested of her care at the facility were another woman’s records. Huge HIPAA violation!! If you love your family member please don’t put them here.

    Would not recommend. My dad was supposed to receive help for two showers a week and we had to battle to get one and he went as long as nine days without one. His room was not taken care of, the food was subpar, most activities were cancelled and none were geared toward men, during COVID lockdown they were just isolated and not even able to visit family outdoors, no dignity at all. The most frustrating part was that when I called (couldn’t go in) I could never get anyone who was responsible for anything. The only time I got a call from the director was when he found out we were leaving (with our $7,475 per month) and asked us to stay. I told him specifically why we were leaving and he said he would check all those things out and get back to me, and never did. We pay a fraction of that cost somewhere else now, and are extremely happy there. Only gave it a 2 star rating because of the location - I really wanted it to work out.

    My experience was having a loved one there for approximately 18 days this past month. It was all pleasant the intake process. The building is very beautiful but don't let that deceive you. Someone tried to warn me about the care before I put my loved one there. I didn't listen as I should have. I would like to give a five star to three nurses on p.m. shift and two aides that act as they cared and was concern about patient care and family care. They are short staff and patients are left in bed and you can hear staff complaints in hall way about the few of them that comes to work are overwork and should walked off. I was there daily and if I was not there another family member would be there. If you put your loved one there, please check on them. You will see and hear a lot. Meal times are always late. I went there several times as they never knew when I would walked in, no aides or just one, or nurse gone awol. No one at nurse stations as you passed by. How can one aide supervise all those patients. During the daytime there are tons of administrative staff in offices and walking around but where is the staff that cares for the patients. One patient walks into other patients room unnoticed all the time. That is not safe . If my loved one has to go back to rehap the Rivers will be a no no. When I tried to complaint they would act in a negative manner. I didn't want anything bad to happen to my loved one. I will also give a five to the therapy team. They were awesome. Fix your patients care team, morale is very low as they need help.

    My elderly Mother had the misfortune of being a resident within the Rehabilitation section at the Rivers of Grosse Pointe from June 6 through July 3. I was with her most every day for 8-10 hours, and in the few days I could not be there, one of my siblings stepped in. Our overall experience with The Rivers left a lot to be desired due to the lack of consistent, compassionate, and professional care for my Mother. It became obvious very early that the Rehabilitation section is understaffed in all positions (Nursing, Technicians, Aids). It is even worse on weekends. This is based on my 28 days and over 250 hours of staying with my Mother to ensure she was getting the contracted services as promised. I give them a grade of 2/5 stars; and it would be worse except for the fact that most of the personnel seem to care about their residents and take personal accountability in their actions in the administration of competent and compassionate care. The problems we experienced were troubling and systemic. Their procedures are inconsistently applied or not known. Several of the employees lack adequate training, or they just do not care. It would be interesting to know what the average length of employment is for staff, but from talking with them there were several that were new or had less than 3 months. In two cases, they were temporary employees from an Employment Agency and it was their first day at the Rivers. Often, there were different interpretations about standard procedure depending on which employee entered the room. This ranged from personal hygiene (adult diapers/barrier cream/bandages/compression wraps), kitchen/nutrition (inconsistency in specified beverages), medication dosing, application of skilled nursing and availability of resident health information. Worse, some staff members would not take responsibility and blame it on someone/something else (another Technician or Nurse, the kitchen, etc.) In one example, it was suggested we inform the nutritionist regarding Mom’s beverage needs essential for food swallowing with every meal, which were then added to her resident profile. Seems simple enough, but nothing is simple getting things done at the Rivers. Even with this information printed on each meal ticket, at least once a day we had to ask this to be honored. I found out it is the Techs responsibility to dress each tray with resident’s beverages as it was received from the kitchen. The blame game went on most every day for this and other issues. I could always count on excuses from some staff members, not the accountability and competent service expected. Response times for assistance of any kind were poor, consistently averaging between 5 to 12 minutes. Upon arrival, they would press a button in the resident’s room console to confirm the request and silence the chime and corresponding hallway light. I grew accustomed to the regular sound of chimes while residents waited for help. I will refrain from calling out the staff by name in this review that were routinely dismissive and rude, of which there were three that come to mind. Two of them were nurses and were very difficult to deal with the entire time. Fortunately, the Physical/Occupational Therapy personal attending to my Mother’s needs were very capable and kind. Thank you, Amanda, Star, Mike, and Bert. Also, there are several Techs/Aids that were outstanding. Thank you, Armani, Carmel, Erica, Thea, Tenisha, Catherine (Cat), and Miss V. The same for Nurses Sade, Shakira, and Jasmin. The Rivers needs more people like this. Finally, I would not recommend The Rivers Rehabilitation Facility to anyone, regardless of age, condition or the availability of a family member or loved one to spend the time necessary for the advocacy of professional and compassionate care. It was a daily struggle with their systemic issues, inconsistency, and quality of care. Quite frankly, it was not worth it as there are other facilities available. I did briefly address my concerns with John Toupin, Administrator for The Rivers. Be the Change.

    Had my mom there for rehab for 13 days . I brought her bottles of water which they never gave her. They never washed her hair or shower. That I know of when I went get her they told me I had to wait 3 days for them to set everything up. Well I took her home and had everything set up in one hour. Not to mention the bruises she got on her arms

    Unfortunately. A friend of mine had to put her love one in this facility to obtain Physical and Occupational therapy. The staff from a local hospital read a review and suggested this place to her and her family to place her love one. This place is definitely understaffed and some of the employees even vented that her on her visits . So WHY accept more patients? Money hungry? Her love one has not being getting the proper treatment that is required to recover. Actually the treatment received has undermined all of the great work the Doctors did at the hospital. Majority of the employees which includes CENAs, Nurses, and cleaning staff could be barely give a DAMN on how our MI love ones are being treated, Hell it's just a job, I guess. People are at their most vulnerable state when families place them at rehabilitation centers with the sentiments and hopes of making a full recovery and yet the opposite is happening at this place! The Patients deserved better and their families expect better. LARA PLZ HELP our MICHIGAN patients and their families get the proper treatment and care they deserve from this facility.

    I am so grateful for The Rivers of Grosse Pointe Rehabilitation Facility. My daughter has been there since June of 2021 and has received excellent care. There have been some concerns and issues, as in all care facilities, but each one that we shared with them were addressed and quickly corrected. I find the nurses there to be kind, compassionate, attentive and patient. The CNA’s work really hard and most of them are exceptionally kind and caring. Also, the Physical Therapy program has been really helpful. The Rivers is one of the nicest facilities that I have ever utilized. It is not only beautiful, but very clean with a pleasant aroma. The management has been very efficient when issues and concerns are addressed, especially with Michael Hilfinger. I have mentioned things to him that were not his direct responsibility but he always makes sure that things brought to his attention are taken care of. I do believe that the shortage of workers was initially due to the beginning of the covid pandemic and maybe people just not willing to work at this time. I’m praying that will change soon. In the meantime, hopefully the management will find ways to manage and lighten the load of the workers. Lastly, my experience with the security has been great. They are very polite, helpful and courteous. Thanks again for providing this most needed service to the community. Luevena A. Dawson

    We moved into the Independent Living Rivers apartment almost nine years ago. Wonderful staff, apartment choices are for 2bed 2 bath or smaller, 3 meals a day or your choice, cleaning services every other week. We could not be more grateful we made this choice

    The Rivers is generally well-run, but you can tell they are struggling at times, especially on weekends. Therapy sessions consistently get pushed back or shortened, and it consistently takes longer than it should to get assistance with things like meals or using the bathroom. The staff always tries to do their best to help everyone, but the staff is clearly stretched very thin. In addition the staff does not communicate well amongst themselves. For example they may not be aware of which patients, are hard of hearing, which patients have dementia, or even their physical abilities. This rehabilitation facility consistently faces understaffing issues, attributing it to various reasons such as the pandemic, weekends, and holidays. Unfortunately, these excuses persist over the years without effective resolution, impacting the quality of care. This review is based on the experiences of multiple short stays. This place was chosen because on paper it looks very good and the facilities are very good but the consistent poor service and consistent excuses are unacceptable.

    I was transferred to The Rivers for treatment and physical therapy after a trip & fall accident first took me to the Beaumont Grosse Pointe ER who diagnosed left leg trauma, admitted me to a 4 day stay and did virtually nothing to treat the resulting agonizing pain..... The Rivers immediately evaluated my needs, accepted and ordered my nurse/wife's suggestion of a Prednisone dose pack to relieve inflammation/pain and began PT thereafter, using ultrasound therapeutic application, after each PT, to heal the affected muscle & tendon. The encouragement to self exercise between PT sessions was invaluable as was the meal offerings which were much better than other healthcare facilities I've needed.
