am
writing
this
to
bring
a
serious
issue
to
your
attention
that
we
encountered
during
my
father's
visit
to
this
hospital.
My
father
visited
the
hospital
for
a
urinary
tract
infection,
however,
his
experience
during
the
visit
was
unsatisfactory.
During
his
visit,
he
went
to
the
pharmacy
department
to
receive
the
prescribed
medication.
However,
we
noticed
that
the
medication
dispensed
to
him
was
not
what
was
prescribed
by
the
doctor
and
he
was
on
that
medication
for
the
last
three
days.
To
our
horror,
the
medication
he
received
was
completely
different
and
could
have
caused
him
harm.
100
pct
satisfied
by
the
doctors
and
this
error
occurred
from
their
pharmacy
department
staffs,
who
will
be
in
their
phones
all
the
time.
I
understand
that
human
errors
can
occur,
but
this
is
not
acceptable
when
it
comes
to
someone's
life
and
health.
Giving
wrong
medication
not
only
goes
against
the
standard
operating
procedures
of
any
hospital,
but
it
also
plays
with
someone's
life.
I
was
shocked
to
see
such
carelessness
and
negligence
on
the
part
of
the
hospital
staff.
I
hope
you
can
investigate
this
matter
and
take
the
appropriate
action
to
ensure
that
such
situations
are
not
repeated
in
the
future.
This
incident
could
have
had
serious
consequences,
and
it's
my
responsibility
to
bring
this
to
your
attention.
I
strongly
suggest
that
your
hospital
takes
appropriate
measures
to
prevent
such
situations
from
happening
in
the
future.
I
hope
to
receive
a
response
on
this
matter
soon
and
that
necessary
measures
are
taken
to
rectify
this
issue.
Right
now,
my
father's
potassium
level
is
high,
and
sodium
level
is
low,
and
he
has
been
admitted
to
the
Thrissur
West-Fort
Hospital
for
further
treatment.
Therefore,
I
kindly
advise
anyone
who
is
visiting
the
Merina
Hospital
Irinjalakuda
to
double-check
the
medication
before
consuming
it.
