    Worst place ever my grandma was sent to this place because her mobility became limited these people had 3 other patients file mixed in with my grandma had been giving my grandma all the medications for those other patients which my grandma became very ill and was sent to the hospital on top of it marked my grandma deceased later had to call the hospital to confirm that she was in fact living with yet no apology from the staff or it’s director this facility is a joke I’m not quite sure how they haven’t been shut down yet when in fact they don’t even have enough cna to cover the patients. We are currently contacting lawyers about the above matter since the director has yet to give us an explanation

    Was in for a easy 14 day respite care stay in December 2021. My 83 yr old veteran dad suffers from dementia and diabetes. He requires supervised attention for feeding, liquid intake and bathroom assistance. After the first 7 days he had lost 6 lbs, suffered from sever dehydration. Was taken directly to lee memorial hospital by ambulance. He also was starting to enter both kidney and liver failure. After this he is completely bedridden and has not been able to recover and has continued to rapidly decline. He has since been accepted by Hospice Home Care program to assist in his final stage of life. This facility is the sole cause of his rapid decline, and the neglect was criminal. The facility is beautiful, and the staff seemed very knowledgeable, therefore there is no excuse or the neglect of the patiencent. Unfortunately we are now forced to plan his funeral much earlier than expected.

    Any of these 5 star reviews are fake and wrote by the staff member and/or the directors themselves. These folks kicked my granny out because I wrote the reviews on them pronouncing her deceased and also mixing her files up with other patients giving her the wrong medication this facility needs to go under a scrutinizing investigation this is a horrible place to send your love ones they’re badly and unfairly mistreated due to their health conditions by the staff members I witnessed myself the staff member vulgarly speaking to the patients and when I asked her is that right for you to speak like that to them I was refused visitation with my granny I witness a patient lying on the floor for hours because the staff refused to help him up this place is horrid

    Ex-employee. I left because they hired a CNA who was violent towards me and put her in a supervisor position as director of activities. She has no clue, putting wigs on rez they don't belong to.I wouldn't leave my dog alone with that husk if a woman. No action by consulate to remove her for her actions towards me. She also has zero schooling for the position. Don't be fooled they ARE STILL CONSULATE HEALTH CARE!! They changed the name so the "smaller" branches would only get sued. I'm sure they'll comment back with disgruntled. Nurses are great there on the plus side. Administrator needs more training on a few things he turns a blind eye to. 2022. Why would I call that number when you obviously did NOTHING Brad. Comes around goes around and sad residents suffer for it.

    I couldn't describe my greatest considerations for this person directly, thanks to her this place functions as one of the best Nursing Homes in Florida. She has dealt with my family's problem in a very professional manner and with a lot of passion. My gratitude and my greatest considerations for her is the ADON her name Yohanka. Thanks to her and her accessories, my family has decent and best care. I have no words to describe the great human being he is and the way he solves the problems that exist with the residents. Thanks once to her!!!!

    Horrible facility my mother calls every day and night crying from how bad they treat their patients. They have left her for hours sitting in soiled diapers and say there is no need to wipe them clean when they urinate. I've called and complained many times and it seems to get worse and worse the more I call. I called one day last week for an update on her care due to her not answering her phone for a few days and was told she was discharged when in fact she was never discharged and her location on her phone (which i track multiple times throughout the day and night) was clearly showing her present at the center. How could someone make such a mistake like that. Nurses and CNAs don't wear nametags half the time and refuse to give their names. How can one be placed in such a facility for their health and rehabilitation and be treated so poorly. If I could give zero stars I would.

    I took my mother there after a hip surgery for rehabilitation and the first negative thing was the nasty front desk lady, she pushed me out of the building don't listen what I was saying or asking and gave me with bad manners a phone number I requested to call and know about my mother, my mother has dementia and I prefer to take care of her at home, they where a full day saying we are working on her paper work for discharge latter I call many times and never had a clear answer or they don't get the phone, what is that place a prision???? They don't let you inn you don't know exactly was going on is frustrated.

    My wife spent three weeks there. The communications was poor. The food was terrible. I had to bring in her lunch and dinner ever day. The juice was concentrate and was sub par . The meals were overcooked and mostly cold or barely warm. There was hardly any communication unless I initiated it. There was no continuity between the different shifts. The staff was poorly trained. The workers were mostly friendly and caring. The physical therapist were tge only bright spot in the vacility but there were a number of therapies that were missed during her stay .

    The staff are great and very attentive to the residents needs/ desires! They truly care about the residents and want their stay to be a good one!! There is bingo, a piano that the residents play, cook outs, ice cream socials, painting.. a lot that the residents do to stay happy! *UPDATE* The good employees/ management have left. The new ones that came in are HORRIBLE! They don’t have a clue as to what they are doing and are rude and condescending to the staff when I was there. The new unit manager Meivys is a joke and hasn’t a clue as to what she is doing! The DON that was there temporarily used profanity at me and when I reported it NOTHING HAPPENED. He left thank God, he was horrible! I wouldn’t trust anyone I cared about at this facility!!!

    DO NOT LET YOUR LOVED ONES STAY HERE. They let a man who is not in his right mind and cannot even stand on his own discharge himself, without speaking with his spouse. THANKFULLY his spouse was able to get there because this hole would have left him outside in the rain. They recommended him to call 911 to take him elsewhere. This is the most mismanaged, backwards, UNPROFESSIONAL facility we have ever dealt with. I am absolutely appalled at how this facility is run, it should be shut down. All they care about is making money. They don’t even answer the phone when you call and the phone number you look up on google is someone’s personal cell phone that is not related to this facility. Do NOT use this facility that should be shut down.

    No matter how many LLCs you try splitting into to hide that this like most nursing homes in Florida are ran by “consulate healthcare” you can do an easy google search to find your own research, what you will find will horrify you fyi we won’t stop until every single name you “try” to hide under is exposed for who you truly are. Look up the consulate whistleblower. We have formed a group to expose every location. You can call yourself happy place fun time nursing home but the IRS money trails can’t hide behind fake names.

    I want to express my gratitude towards the ADON yohanka because the whole time I spent their she's treated me with alot of respect and patience she is very professional in her way of speaking to family members and in giving out clear answers to my problems I'm very thankful she is working here !

    I just have to start out by saying I profile this place before I ever went there and I'm very ashamed of that because let me tell you I couldn't have been more wrong and I'm glad to be so. This is the cleanest facility that I have ever seen I have been in a lot of these places with my mother and father and grandfather. I have never seen a place run so well this place is top-notch it is the cleanest.smells great..food is good. The speak to my father Ralph Anderson with respect and compassion. If your loved ones are in need of a place to go let me tell you your heart and your mind will be comfortable if he is there because I know now for the first time in a long time mine is.

    Memory Care Unit = adult jail 👎🏼👎🏼👎🏼 (second review after less than two weeks). Evans Memory Care Unit has three people on duty for a total of 26 patients, and the three are not guaranteed to be inside the unit; Typical of Medicare and Medicaid, a shameful institution for our elderly and veterans! Mom has had a wicked UTI, that began before she transferred to Evans. Almost 2 weeks have passed to get medication, which is unacceptable!!! Water is only given at meal time. Water is not allowed in patient rooms explained by one of the aids, “because the cups tip over, the water is a hazard, and the staff cannot keep the patients from drinking the other patients cups.” Mom is completely dehydrated every time I visit. The other Memory Care patient’s are all in different stages of their disease. They wonder in and out of rooms as does my mother. Every time I visit, other patients are right on top of us, touching inappropriately, listening closely in on any conversation and unresponsive when asked to move away. There are no boundaries there whatsoever. Mom has seldom been combative but has been in her time at Evans. Today, I was asked by an aid, if I wanted to change my mom’s depend and clothing because mom won’t let her. The aid did change mom’s clothing after I said no, asking that she do so. My heart broke today when mom told me that she is praying to God to die. I don’t know what the answers are but I continue to advocate for mom, and I trust that God has a plan in all of this nonsense.

    My father spent 4 months here and I visited him daily for 6 weeks- the wound care is excellent, his room was clean and he seemed to think the food was adequate. The staff were all very nice and kind and seemed to care. I was hesitant because the reviews were rather poor but upon spending time daily observing I think this place deserves consideration.

    I had a customer that was at this facility and due to the lack of attention, he had an infection is his rear from sitting too long!! This infection reached the bones and had to he operated. The surgery was so severe that ultametly killed him! He was an 83 year old healthy gentlemen that ended up at this facility from a car accident!!! Very sad! Find a different place!

    This opinion is directed directly to the ADON of Evans, her name is Yohanka, a person with a lot of passion and attitude in her work and everything she does in reference to this place. We all love them very much since they are an example to follow and all their dedication. She is a good leader and I think that place works very well with her present. I give her a 💯
