    HORRIFIC experience for the 5 days my father was there! All the staff (RNs, nurse manager, nurse assistants, as well as the administrator of the facility) we encountered were incompetent and neglectful. They accepted my father for hospice care even though they are clearly not set up to provide that level of care but I presume just wanted the money that we had to pay up front. The nurse manager admitted to me (after several days of my father being there) that she was not familiar with end-of-life signs and symptoms which shows they should not have accepted a hospice patient. Although clear and written orders were made by the hospice nurse from an outside facility, medications (including those for pain) were not given as ordered and other basic hospice care responsibilities were not provided as ordered either. Once my father passed, his belongings were never sent to the funeral home with him and when I called to speak with staff, no one knew what had happened to them (including an expensive pair of hearing aids). I have left a total of 5 voicemail messages over the course of 4 weeks for the director of the facility stating that I would like to discuss my concerns and have yet to receive a return phone call or communication of any sort.

    My Mom was here in November and December of 2020. Of course the facility was locked down due to Covid. I asked for a weekly update and finally got 1 after leaving numerous messages. The nursing staff and social worker could not answer specific questions very well. I did however receive weekly calls from a corporate office telling me how many new cases Covid were there. It was always the person never came in contact with my Mom. One day the social worker called out of the blue and said my Mom was not progressing and she needed to go home. I had to scramble because she was bed ridden due to their lack of rehab. They ended up sending her home with Covid, sepsis and a stage 4 bedsore. They never mentioned any of this to us. Now 10 months later we are still trying to get the bedsore healed. Do not send a loved one here.

    Under new management. Disregard what negative stories you have read or been told. Firstly, facility very clean, overall pretty modern and yes some rooms need some updating but very clean! Food is hospital style like, but no complaints what was presented to my sister! Food dietician came to visit often alerting my sister what she should be eating and sugar awareness level. Care excellent, never met some many wonderful caregivers. Really compassionate and kind. Office staff knowledgeable and helpful. Social workers very kind and understanding. Rehabilitation staff the best! They really care and teaching my sister safety care and exercises to strengthen her core area. I urge you to call and visit/take a tour and sure you I’ll be pleased with the facility. I would love to give a shout out to the folks listed as no better way to thank them all and has made my sister’s visit a positive one. (Heather, Deepak,Martha, Susan, Lindy, Breeana/Kim, Omar, Val, Amy, Andrea and all whom have helped my sister get well!

    I don't recommend this place to anyone, I have witnessed countless forms of workplace abuse and no one is ever held accountable. They are so unprofessional specially Rachel Ames(HR) and Heather Lynn(DON). I have reported neglect and abuse and nothing has been done. They are not properly taking care of the residents, their beds are always soaking wet and with feces. Staff suffers constantly because they are always short staffed working with 3 CNA's on the floor with 40+ residents. Management never goes to help on he floor not even to pass a meal tray, all they do is walk around and sit in their office. They violate HIPPA constantly talking about staff's business to each other. CNA's are belittled. Human Resources (RACHEL) never has an answer to any concern we would bring up, she doesn't know how to do her job and is so rude, unprofessional and an airhead. It's a very toxic work environment and had made co workers scared to speak up because they're afraid action will be taken against them.

    My father came here for physical therapy and rehabilitation after a stroke. After being released he was frail and had to be admitted to the hospital for health reasons that went completely un-addressed here. Instead of admitting him to a hospital, a lift was being used to move him around to "prevent" the health issue from occuring rather than a doctors expertise. We found out his health issue could have been fatal and the doctors were disappointed that he was in that state for such a prolonged amount of time. He also deteriorated physically in his time and was not able to even stand for 3 seconds when leaving the facility. He did not receive the care he was admitted for unfortunately. A better location isn't always a better option as we thought in this case. The staff seemed more unwelcoming than warm. We would schedule our visits and multiple times denied our visit being told he was in dialysis. Mind you the visits were different days and times which are inconsistent with his consistent dialysis schedule. Something was very off here but they're very good at keeping that hidden. They may be better with success on less dependent patients.

    My grandmother was recently discharged from this facility. Everything was great from the care, to the food, the therapist, and of course the visitation. I was able to see grandma the next day after admission.! This helped with her adjusting to the new environment as she suffers from anxiety. I am so thankful for all the care she received. Thank you ManorCare Libertyville for providing excellent care to my family. Keep up the great work!

    I had a terrible experience at this nursing home. The staff was incredibly rude and dismissive, showing little to no concern for the well-being of the residents. The facilities were outdated and poorly maintained, with dirty and cramped living spaces. The food was practically inedible, and it seemed like the staff didn't care if the residents ate or not. I saw several instances of neglect, including residents being left in their own waste for extended periods of time and not receiving the necessary medical attention. It was clear that the nursing home was severely understaffed, and the few employees who were there were overworked and visibly burnt out. I would not recommend this nursing home to anyone, and I strongly urge you to look elsewhere for better care for your loved ones.

    My family and I are so very grateful for the incredible care and service our uncle has received during his recent stay at Manor Care. Staff are caring, informative, and prompt as this was the first time we had a loved one at a nursing facility. All the staff went above and beyond to provide him with excellent care! I had only had one concern which was addressed by management immediately and since then I get weekly calls providing me with updates related to my uncles care. Communication has been great and the compassionate care visits and now outdoor visits are so helpful for his emotional well being. Thank you for all your hard work, Manor Care staff.

    Agency Nurses and cnas be aware HIGH cancellation rate, and then they dnr if you cancel on them to all of their sister facilities, don’t be confused by the call out phone lines I notified them of an emergency with my daughter before my shift and I got dnr. Mind u every shift I picked up with them prior that week they kept canceling I would pick up anywhere but here. As far as facility typical trash, staff rude, unsanitary, no one introduces themselves but ask u a ton of questions. Family be aware many agency are present so your loved ones are not receiving continuity care Sincerely your honest Nurse.

    This is no place for any of your loved ones, I would not put them in this abomination of a nursing home, where if you are in need of a hospital you will not be admitted to it. Also the staff that were encountered were neglectful, incompetent, and unwelcome toward guests and patients. I wish there were good things to say about this facility but it is hard to provide such information when things as necessary as food are deemed nearly inedible and a safety hazard. Here you will find that they will fail to provide you with necessary information about your loved one and the possibility of them coming home with an undetected disease is very alarming.

    Second review. Just as an update. My mom and I had a more pleasent experience after speaking to the social worker and a couple other people. My mom loved the therapists. They were very nice to me as well. After the first few hiccups things changed. Yes still some hiccups but all in all not a horrible experience. So I will recant my previous comment. Some bad staff but mostly good staff members. However this is anywhere you go. I would not recommend this place at all. I dont even want to give them a one star. This is my first experience with a rehab center. They take my mom for physical therapy once a day. How is she suppose to get better. I asked on Tuesday when they take a bath. They said Wednesday. Wednesday came and went and we asked again. They said in the evening. Well apparently the evening never came because no shower no wash down. She asked for them to help her sit at the edge of the bed and it's to much work. According to them this is a day shift problem. Nobody knows anything because they are all agency workers. They push work off to others. Oh I was going to use my mom's bathroom and it smells in the bathroom, the bed pan was over the toilet. I moved it and there's waste in the bed pan, in the toilet AND ON the toilet. I really wished I could move her.

    From my first night here the staff have very courteous on helping me obtaining my goals. The cna's were on it when I needed something. The nursing staff were great and were on top of it when it came to my meds. The OT and PT staff worked with me and even pushed me to to do more overall very good place

    I want to say that this is one of the Best places I have ever worked! I have learned so much from working here. It was time for me to move forward in my career but I want to say your DON Heather is the sweetest most loving person, and Carolyn your HR is THE BEST not to mention all the great CNA’S I worked with ❀️ I will miss all of you!!! Best Wishes, and keep taking GREAT CARE of OUR RESIDENTS ❀️❀️❀️

    ProMedica is a great facility. I’ve had experience with the prior management when it was Manor Care. In 2017 they were fantastic and my fathers stay there resulted in him gaining strength, mobility, and confidence with their caring and knowledgeable staff of nurses, therapists, and doctors. In 2020 I had another experience with them (as Manor Care) for my father and at that time, at the start of the pandemic, it was not a good experience. I will not elaborate but I will say at that time there were many problems which I believe were the result of poor oversight and management as well as staffing shortages and the Covid pandemic preventing family from visiting/checking in on loved ones. ProMedica has changed significantly for the better, is under new management and has undergone quite a transformation. My mother was in a terrible car accident and required skilled nursing assistance along with extensive rehabilitation. Our options for that level of care in the area were very limited. We chose to take a chance with ProMedica knowing it was close and my sister and I could be there daily to check on her. Choosing ProMedica was the best choice. My mother has thrived and healed under their care! She was on a ventilator and feeding tube upon arrival and could not walk, eat, talk, or do basic self care. The nurses are fantastic! Kind, patient, caring and well trained to manage complex health issues. With the help of their Physical and Occupational therapists my mother can now walk and do self care. Under the watchful eye of Dr Baker, and with the assistance of their Speech therapists, my mother was safely transitioned off the ventilator, her throat and neck became strong enough to talk and soon after to eat. We are bringing mom home this week and are happy she is ready and feeling confident about returning to her normal life. The social workers helped work with insurance and arranged care conferences where we were able to collaborate on goals and obtain more detailed information on her progress. The facility coordinated follow up Dr visits with specialists and arranged transportation. The entire staff seemed invested in my mother, our family, and our wishes for her care and rehabilitation. 2 years ago I would not have imagined posting this review. But here I am today happy to share my experience. ProMedica has been an amazing advocate and partner for my mother and our family through a very difficult period. I am grateful to everyone at ProMedica and I firmly believe they are the reason my mother has recovered so well.

    My MIL is there for rehab. The place was clean and did not smell at all. The residents seemed happy as I talked to 3 during my time there visiting my MIL. My MIL was less than impressed. My observation was that care was being given by the nurse, CNA etc as they stopped by frequently. However none ask if she needed to use the bathroom or if they could help her with anything, just polked their heads in. My 3 star is because my first visit my MIL is still in bed at 4pm. Not sure why and the manager was not there to talk to me. I don't want her sitting in bed all day because the little strength she has she will lose. Not good at all. I am going back tomorrow to talk to the manager about getting her out of bed and walking with her. Her physical therapy is for her arm, shoulder and neck..walking can keep her legs strong while they rehab her upper body. Will also have photos tomorrow.

    I am getting ready to DC later this week from hip replacement and could not compliment this place enough. Big shout out to my therapists Omar, Danny, Jen, Linda and especially Chiraq. Very knowledgebale, sweet, patient, and helped me get stronger. Another shout out to the kitchen & nursing staff. Great smiles, meds on time, hard working. I was well taken care of because of all of them.

    When my mom had to leave her prior medical rehab facility with very short notice on May 1st, we moved her to Avantara Libertyville. We were welcomed to the facility by Cecilia Estrada in admissions (who is super nice) and she ensured my mom got settled and her information was processed quickly. This is a larger facility and the resident rooms are very spacious and comfortable. The selection of TV channels was better than at my mom's prior rehab as well (which is very important to my mom as well as the other residents there). While my mom unfortunately had to switch rooms after her first week there, Heather Armon, Director of Nursing, personally ensured the room transfer went very smoothly and made sure my mom was comfortable during the move. My mom was mainly at Avantara for wound care, as we have been working to close a sizeable wound for several months, and Marina the wound nurse took great care of her. When we were ready to transition my mom home, Kim Bonhomme, the social worker, ensured everything we needed was ordered and delivered to my parents' house in advance of mom's move home. Liz in wound care even provided my mom's new fulltime home care givers and my dad with general education on her wound. Finally, after we got mom home, we had a few questions about her care and that arose and the team at Avantara met with us again to ensure that we had all questions answered, and knew how to take good care of mom. We very much appreciate the personal attention we got at Avantara and if the need arises again, we would not hesitate to use them again!

    I am a long-term resident at Avantara, and could not say nicer things about the former administrator (Karli), and new administrator Victoria, the Director of nursing (Heather) and Sherry at the front desk. They always make time to joke around with the residents and will always say hello with a smile. I have enjoyed my time at Avantara! Mike from Central supplies is cool and the ladies from Activities make me smile. Cecilia the new Admissions Director always seems happy and enjoys listening to music with me!

    I have been here since March 2023 and have enjoyed my stay so far. I am a long term patient now on third floor. All of my CNAs have been very nice but especially Fawne and Evelyn. My usual nurses are Meghan and Juan and they are very nice to me.

    Zero stars. My grandmother became very ill from this place. They moved her from the 2nd floor to the top floor because she went to the hospital and someone needed the room without consulting family first. She then became ill and passed. It smelled bad on the top floor. We complained to selin the supervisor and another but they did not help. They lost some of her clothes when moving rooms. A overnight staff from another place removed the call button from my grandmother and closed the door so she does not bother her. When my grandmother did physical therapy a cleaning staff member (lady) made fun of my grandmother saying she was pregnant because of her stomach when she has a tumor. Elderly people being abused is a real thing and should be stopped. There's patients who cannot speak and it's unimaginable that people can be cruel to the elderly.

    I am a current rehab patient on second floor and have really enjoyed my stay. I have enjoyed working with Alyssa from therapy. My CNAs are very attentive and always treat me like family. I would reccomend this place highly and would come back if needed in the future.

    Working here has been a pleasant experience. I’m in The Activities Department and I enjoy doing crafts and popping popcorn once a week. I enjoy the work environment and I am honored to work here.

    Very poor service if you have a loved one who's in their late 80s or in their 90s. With exception to one or two nurses, they just didn't seem to care. My dad was struggling to eat or swallow plus he's very weak, and they gave him food he needed to cut and chew. They gave him these finger bread rolls, which are hard to chew and swallow. They never tried to help him or never said anything to us, unless we asked. We started visiting more during meal time to try to help him eat and drink. We also had to check is drawers to make sure they changed his adult diapers, because they would just leave him in it. My dad went downhill while there. The day we were moving him out, we found out at the moment of moving him, he had a 104 fever. They never checked him. They bruised him trying to make him do rehab exercises and his arm continues to hurt. My dad had covid and was placed here by Condell (don't get me started on them) for a 2 week quarantine. Probably the worst two weeks of my dad's life. They need to be more humane as well as listen to the patience family, instead of arguing with us. Hopefully my dad will recover some before he passes. My dad is also a veteran and they treated him this way. Just ridiculous care.
