    New doser. My first week in and I was never given a "blue book" I wasn't aware that the dosing time on Saturday was different then the rest or the week and when I went to get my dose before work I was told it was too late. No exceptions even though I'm a brand new doser and even though I was never given the book. Thank you for making me potentially lose my job. I kinda need a job to you know.. live? Very unprofessional and rude. What's the point of being open 24/7 when you can't even dose after a certain time ? Ridiculous and extremely disappointed I will more then likely be switching to CMS .. Adios !

    If I could give this establishment minus 20 stars, I would. I am actually considering reporting them to Arizona Medical board or psychiatric board if such thing exist. I will be looking to get my son to Banner or North West, and that, despite liking his current practitioner at CODAC. However, CODAC is one of the most horrific places for people with disability who do not have the mental capacity to advocate for themselves. The thought that one day I would not be present and this place would be the psychiatric provider for my 20 year son with moderate to severe autism, truly terrifies me. For those you who are guardians, unless you have no other job than being on hold at least 40 minutes or more, take a message that they never reply to, and you can keep trying, this place is a dangerous establishment. The staff lie about contacting you back, they do not answer calls, they lie about emailing you and when you complain by calling their solution line, they lie and do nothing too. I know this because I do search for their email in my inbox, junk mail box and trash and there is nothing from CODAC. It seems like at CODAC, lying is the culture of this institution. If you have a loved one for whom you have guardianship, please look elsewhere for care. Mental health is not only about having a competent practitioner, but having adequate access to make appointments with your practitioner, get the needed prescriptions ordered ontime, and have your inquiries responded to regarding making appointment. This establishment's employees don't even know how to use their own patient portal. When I recently contacted them to make an appointment for my son, they replied by asking; "Do you really want this appointment?" on the top of this very dumb question, please note that their email system has disabled relies, so you can not answer the stupid question. In closing CODAC operate with as much technological efficiency as mid-to late 20th century, without a fraction of the ethics of those times.

    This place is an absolute joke and waste of time/money/resources! If you want to go through hoops and try calling staff/MAs just for your voice to NEVER be heard this is your place! Been coming here for over 3 months and up until today was the first time after reaching out from the start that my recovery coach actually called me. On top of that the NP Adam Case in his bright mind without ANY warning stopped a prescription I was on for 7yrs (benzo) to where it sent my body into complete shock and withdrawal along with only being told “we don’t know why he did that.” If you’re looking for staff that truly doesn’t care and a place where you get ZERO help this is the place for you! Good luck and keep searching for a better MAT clinic because this one truly is the worst!

    Been going here for methadone for the last 4 months. They are nice people and they're caring kind and helpful. They are understanding and willing to help people many ways. They help you get medical and mental care they help people get employment and sign up for schooling and classes. They don't just offer methadone and suboxone. They help you get on your feet and through the recovery process and starting back on a normal path to a safe and sober life.

    Though we had some discussion about discrepancies during my intake procedure, I have received good RX refill, counseling, housing counseling, medical, and feel I have a friend. My problems aren't substance abuse related, but moderately serious. I am not on the recovery coach plan; I have to make an appointment with someone on call to obtain services. My friend who is a nurse felt that I need a recovery coach.

    Caring staff that try their best. I upped my review from 4 to 5 to counteract all the negative. A lot of the zero star reviewers simply put were or are not ready to heal. It's everyone else's fault. CODAC saved my life. Period. Pic is a good friend and my son at the clinic. God Bless them all. Happy Holidays CODAC!

    Too bad zero stars isn't allowed. Adam case is amazing as a provider. I suddenly was switched because my new doctor, Angela Brown decided to leave (I now realize most likely for great reason.) and I was switched against my will to a provider named Daniella who called every single one of my other providers to let them know that I was on suboxone. Bent and stretched HIPPA as far as she could... After being completely off of opiates for well over 2 years, not failing any drug test, she has me coming in once a week and constantly Is late calling in my prescription and so I am constantly sick And this is absolutely inhumane. She could get anyone to relapse very quickly. Ashley and Carmen at the front desk are absolutely amazing and do everything they can to correct doctors like hers complete negligence of her patients. They preach that protocol is Bible, however she has not seen me in almost 2 months. Codac does what they can and she does the opposite. I would be surprised if half of her patients do not relapse constantly.. It's disgusting.. Stay away from her like the plague. She knows for a fact I have extreme health issues outside of opiate addiction years ago and and switched me from monthly tests from my last provider to weekly tests for absolutely no reason when she knows I have no form of transportation and I have been through extreme abuse recently, police files brought in and on record in the last month as well as immuno issues where I can barely get out of bed. Again, stay away from her like the plague. At this point, I would recommend cope or hope. Shame on her. Shame. On. Daniella as a provider. I am contacting the medical board as soon as they open. What a joke joke of a provider.

    2022 I am a member at Codac and just wanted to take a minute to say that Ann, one of the dosers is such a beautiful person. She has helped me in several ways in the year that I have been there. I just want to say thank you to Ann for her help and conversations with me. August 2023 Emma Mangen is my new recovery coach. The one before her was recently let go. However, Cheyenne had the paperwork done for me weeklies and they were "at large". Emma right away had me go in and take care of it. She was very new to Codac and of course I was concerned. Emma handles everything like a champion. Easy to talk to and be your true self. She was a blessing in disguis. Very accomplished to the point that you would never know she was new. She made me feel special. Professional yet extremely kind and patient. It was a nice feeling knowing that she was in my corner. Emma is one of those employees that is a keeper. You don't want her walking out the door. Being new and doing that well who's to say where she is headed with Codac. Thank you once again Emma for the time you spent with me and for me. When we stood up I couldn't help but to give her a hug. I'm not used to people being there for me and to say the least I am most grateful. With true sincerity, Sonny Rubey

    Very Rude receptionists, the security guards are more professional then they are. On the 6th day of treatment The nurse made me aware my dosage had to be increased daily and I was supposed to have had an appointment with my primary already, he apologized and told me to go on over the reception and ask for an appointment. They called me the next day and the lady on the phone confirmed that in fact my dosage was supposed to have been getting increased by 10mg daily. Thank goodness for that nurse that made me aware durin dosage hours because I was having a real hard time with the symptoms and my wife was getting ready to take me to a private clinic for better treatment before I gave up on recovery.

    I started my treatment there 67 years ago and it was nothing but a open drug market outside of the place . The rules there didn't make any sense and would make r a new comer turn the other way . I went to Community Health Services and never looked back.That is how a Mat Clinic is supposed to be run. They don't judge you're CODAC does and treat you with respect unlike CODAC . I have sent a few people there a long time ago and they all relapsed . they came to CMS and are doing way better .. this place should take lessons frOM CMS ON HOW TO RUN A CLINIC unless your still using then go to CODAC . also dear friend of mine came here and right on property he bought benzos on top of methadone. he od and died 6 years ago due to there negligence. sad really the staff knows whats going on there . too bad I couldn't get a hold of him and take him to CMS with me he'd be here today .

    Had one appointment, and then my 2nd appointment was literally cancelled without explanation at the the exact time it was scheduled. I have called for days, and get put on hold for hours. Stay away from this place!

    Tbh, this place saved my life. Was in the methodone program for 18 months and mostly had good experiences from there. A couple of the recovery coachs might need a little help but overall this is a good place. Also big plus, I was dosed pretty quick after I started intake. If you really are serious about getting clean this place can help you.

    I am writing this review today because I am lucky to have a VERY caring and proactive team caring for me here at CODAC. Adam Case is my psych provider, and he is simply amazing! I really detest having to attend appointments, in general, but I actually look forward to my appointments with Adam. Adam is not judgmental, he is a careful listener and is always game for helping me make adjustments to my meds, as needed. Adam has a great sense of humor and NEVER rushes me through appointments. Adam is always encouraging, but he hears the unspoken things and follows up with me more often when he “hears” things that may raise concerns. I greatly appreciate the fact that Adam is a very genuine person, who truly cares! My recovery coach’s name is Sonya Castro. Sonya is also an amazing human! Sonya is always prompt, never leaving me to wait. Sonya truly cares and always surprises me with options that I never knew were a part of her job. I actually don’t think certain things are even a part of her job, I believe she is just ready to go above and beyond her job description, to help me with things. I am going through a very scary and dark chapter of my life right now, and Sonya is a light in the darkness. I feel safe and cared for. I am SO very grateful for Sonya. I feel like my load has been lightened, because Sonya has offered to carry it for me, allowing me time to breathe. Of course, there are many others at CODAC that deserve recognition as well. Another person that has proven he cares is Joe. I don’t know his last name but he is always a smiling face and treats everyone with kindness, including my 11 yr old dog, Mr Ike. He refers to himself as uncle Joe. When needed he is always willing to hold onto him for me. My dog loves him and we all know dogs are the BEST judges of character.     Today was successful 😃 I met with my new primary care provider and got a C+ health grade " CKD level was 54" but got a A+ on my Suboxone treatment requirements. Today marks Days: 159 Weeks: 22 Months: 5 Hours: 3,816 sober and I don't have any cravings or anything from the drugs and alcohol I onced used.. Exactly 5 Months I did a entire 360 degrees with my life and it's only getting better as the days go by and I thank God, CODAC Health, Recovery & Wellness, Inc. & Hope Inc without these guys I probably wouldn't have gotten this far along with my sobriety journey.

    I have completed multiple courses with codak. I have a huge amount of respect for all the counselors and staff. I had made a complaint a few weeks ago about the people who were answering the phones overnight. they didn't represent codak's values or there mission statement.I am sad to see that you guys no longer have a 24-hour call service. I hope that you can find staffing to make that available again and I thank you for all of the great lessons that I've been taught from the classes you provided.

    I'm not sure what happened to the quality of Heath care but I can see they're trying but offen time fall short and the substance abuse treatment is poor at best many of people who I have known seeking help become disenchanted with the personal at this facility.

    Mr. Bond and Ms. Acuna came and delivered Narcan to me, and they made such a positive impact on my life! The officer who was going to bring it to me got caught up on the field. They brought it in his place. The whole interaction with Mr. Bond and Ms. Acuna was healthy and comforting! If I ever need them again, I know where to find them! Thank you guys for what you do! Keep up the great work! I am very proud of the two employees I got to interact with! I hope every employee is like them! Then CODAC is definitely doing right and succeeding in helping their clientele! God Bless Us All!!!

    When you have the opportunity to see them, the staff are friendly and professional. You WILL be on hold for an hour or more trying to speak with anyone on the phone, but if you have the opportunity to actually speak with the staff at the front desk, they're lovely. Dr. Steve Lee (therapist) is a wonderfully compassionate and thoughtful person. I always felt heard and seen when I spoke with him, and really felt free to be and express myself. He's very open-minded and you can see the compassion he has for his clients.

    This will be the 3rd time the staff during the week has turned me away because 2 weeks ago I went to another clinic which I'm no longer a client there it's been 2 weeks there is no active prescription I have to see a doctor at either places to dose my insurance sure won't let me or pay for a double dose there are many other things in place to make sure this doesn't happen and it's not the front desks job to make sure the law is enforced!! They will turn you away and tell you that you have to wait 2-3 days because of some faxing situation which is ALL NON SENSE just don't mention the other clinic at all if you were enrolled somewhere at some point in your life DO NOT TELL THEM they will require all this documentation before they even let you see a doctor.

    I’ve been treated by Codac for many years and I give the staff 5 stars ⭐️. Everyone has been professional, prompt and kind. Codac is not the only clinic I’ve been treated by but they’re definitely the best.! Thank you CODAC
