    Just awful! Unfortunately my mother had to stay at Signature Healthcare in Port Charlotte for rehabilitation. Luckily she’s of sound mind, I can’t begin to imagine how they treat other patients who are not. She had money stolen from her purse. I called to speak to the Director of Nursing, never received a call back. Aides are rude and only do the bare minimum. One time I was visiting at her window (due to Covid you cannot go in) her blinds were closed. She was pressing her call button for assistance, took 15 minutes for someone to acknowledge her. Thank goodness it wasn’t an emergency. I can say Bruce is amazing, thank God for him. He is kind, caring and helpful. Great job Bruce! Due to the facility rescheduling appointment with her orthopedic doctor, an infection went untreated. Her orthopedic had to admit her to the hospital and perform 2 surgeries in a week to remove infection, picc line antibiotics for 6weeks.If the infection went any longer it would have compromised the hardware from the original surgery. Would have had to have hip redone and restart recovery. Due to the negligence of Signature Health Care she was moved to another rehabilitation facility. Added photos to show the quality of meals served.

    When it comes to helping a fellow human being who can no longer help themselves, a few words come to mind. Compassion. love, care and empathy. This establishment possesses NONE of those qualities. What I witnessed over the past seven days from Charlotte Bay Rehab & Care Center has been nothing short of horrific. It's a dumping ground for the helpless and forgotten. You do not go here to recover, you go here to die. If you truly love your family member or friend, I beg you to find other rehab/care services. Their website is a ruse, and completely misleading. From the moment you walk into this place you smell misery and death. There is always a faint urine and fecal smell that permeates the hallways and patients are dumped in those hallways, some crying and in such despair. They are left there to sit in their own fecal matter and urine soaked diapers. I implore you to avoid this place like the plague. I could go on about the cruelty and mistreatment from this place but I'm so disgusted. I have no doubts this place will be shutdown once the state of Florida steps in and law suits are filed.

    The chaplain Dennis Moreno. provides the spiritual support that is so very much needed by the residents and family members. He is a much needed beacon of light in this facility . As with any nursing home / rehab center there are up sides as well as downs The upside is this place has been recently updated. The upside is this place has been recently updated. There's also a very good physical therapy room with specialists The downside is, if you have a loved one with behavioral or mental issues, I do not recommend signature healthcare in fact I do not recommend the state of Florida for anyone with either of these issues Florida has been ranked at the bottom of the countries list for counseling or treatment for either. So, if you're going there or sending someone there for physical therapy this is the place! Otherwise stay out of the state of Florida

    It’s places like this that give care facilities such a bad name. My brother has been at this facility for a couple weeks now and enough is enough. We’ve moved him to another place. He hasn’t had the ability to make it to the bathroom on his own and they’ve left him for very long periods of time laying in his own mess on more than one occasion. The staff has proved to be neglectful, unsanitary, and rude. The food has even been served cold and is absolutely awful in every sense. The place is under staffed and most likely underpaid. If you decide to place your loved one here, you better keep a very close eye on the situation!!!

    Were do I start, my brother was moved here from the hospital for for rehab, his muscles needed work. He spent 10 days here and go so much worse, we finally transferred him out to a hospital where he was diagnosed with two blood clots in his lungs, and pneumonia, and a very large bed sore. The hospital thought that it was all due to lack of movement. This facility was supposed to turn him on his side every 2 hours, they did not. They were supposed to change him so that he did not set in soiled garments, they did not. They were supposed to make sure he stayed hydrated, they did not. They were supposed to feed him healthy meals to help him start gaining his strength, they did not. they were supposed to make him exercise everyday, they did not. They were supposed to help him get better, they did not. If you truly love your loved ones, please do not take him here, if you do, and leave them here, he will die. Since my post, I see some positive reviews. It is sad when this place starts to have their employees leave review, this says a lot about this place. They can’t rely on customers, so the have to pad their reviews.

    This place is terrible! I would NOT recommend this facility to anyone in search of care. My father was a patient here and from my personal experience, it was very untidy, the staff was basically nonexistent, he was instructed to have 5 days of PT per week and he managed to only get 2, the lack of awareness by their employees was absolutely absurd, and not to mention, their idea of PT was handing a man with little to no strength a set of workout bands and saying "have at it". If you're considering this facility for you or a loved one, don't. You'd be better off looking up Youtube videos and managing the care yourself.

    My grandma was there less than 8 hours and fell and broke her ribs I would not recommend this facility to anyone the staffing does not know what is going on at all I was asking out on the patio with my grandmother if I would come and fix her bed and it wasn't even her room and I was the wrong person they were looking for they never even knew my grandma was transported to the ER from the facility I would not recommend this facility 100% to anybody if your family is in there I pray you pull them out or I pray for him every day and every night 100%

    We are new at Signature Health Care in Port Charlotte, but the staff has been wonderful, friendly, and very personable. I’m very glad we located signature for my husband to be well taken care of. I specifically want to mention the chaplain, Dennis by name. His love and warmth spreads all around and he radiates with welcoming arms . So glad we are here.

    Signature of Port Charlotte is a wonderful place for your loved ones. The staff are AMAZING! I can not express enough on how much Dana & Michelle in Social service have helped my mom while dealing with the loss of her eldest daughter. Not only SS but the whole staff have comfort her through this difficult time. My mother receives excellent care and she loves it ! I would like to thank Alec the administrator and the whole staff for everything . You all ROCK! Sincerely Kendra M

    Staff are wonderful, I can request the food I want. The facility is clean, house keeping does a wonderful job. Chaplain is wonderful, Dennis always gives me & other patients hope. He encouraged, me back to faith in God. Inspirational time is awesome. When your alone, he is a friend to us.

    My neighbor Peter needed a rehab facility. I found out about Signature Healthcare. I went there spoke with employees, as well as patients. I walked around, checked their rehab facility and was very impressed. I also met and spoke with the Chaplain. He was very accommodating and was happy to show me the rest of the facility,(very clean and everyone seemed pleasant). It ended up, Peter went to Signature for rehab, was very satisfied. As of now, 2 months later can walk(95 years of age) and now is at home with his wife and both are very happy. My sincere thanks to Chaplain Dennis for all the special help and attention, that he preformed for Peter and myself, giving me relief with all my concerns. I will always recommend this facility.

    Place is terrible my dad came in from Fawcett hospital with hip surgery I told nurse he needed his pain pills they said no that a doctor had to see him and I couldn't bring his medication and nobody came to take care of him they left him in pain not recommended to anybody

    This facility is unsafe and should be closed for good. The lack of care that my loved one received at this place may cost him his life or at minimum permanent damage to his health. Places like this cut every corner and take all the shortcuts. They are profit before patient care. They hire staff that should be band for life from healthcare. The lack of care provided to a patient should be not only considered neglect, but if done purposefully should be labeled as assault. All levels of staff all the way to the owners of places like this should be held responsible. All healthcare professionals should treat every patient as if they were their own family member. Im truly disappointed of what healthcare has become. Its hard to be a healthcare provider now when you are expected to worry more about cost of care more than providing good care. This facility is just that. It deserves to be audited and charged with all its fraud and miss treatments. Please find a new place if you have a loved one placed here. There are better choices. Do your own research and dont always trust those whom discharge your from the hospital or the information you recieve.

    ZERO stars! This facility is horrible! Do not leave someone you care about here. The food is terrible. This place was Horrible. The nursing staff inattentive. I don't believe anyone is giving this place 5 stars . If I could rate a 0 I certainly would have. This place should be investigated and shut down. Call buttons going unanswered leaving patients to soil themselves because they need help getting out of bed to use the facilities. Inmates get better treatment !

    My uncle was brought here for rehab from the hospital and was totally neglected. he ended up developing pressure ulcers after only having physical therapy for 4 out of the 11 days he was there, and was readmitted to the same hospital with bilateral pulmonary embolisms and pneumonia. he also had a central line for antibiotics which maintained the original dressing from his hospital stay for the duration of his rehab stay. we asked for the contact information from the rehab facility for upper level management after the manager failed to rectify our complaints during his stay, and the manager said they do not give out that information to patients or family. they do not care about their residents and it’s apparent when you go to visit a family member and there are 30 call lights going off with people screaming for help from their rooms, meanwhile 8 employees are joyfully conversing at the nurses station.

    I work for Signature Health Care for 8 years, I wanted to appreciate a compassionate person. Few months back, I was so sick & delirious. My family did not know what to do. Our Signature Chaplain, Dennis came to check on me. He prayed, encouraged me & my family. He asked them to take me to the emergency ASAP. I almost did not make it, if not for a caring & compassionate Chaplain. Thank God someone cared. I believe Signature cares for their staff.

    My husband was brought in here 2.5 weeks ago and has had excellent care. His birthday was special because they had him in the family room in the wheelchair when we all came in. Also they take good care of his hygienic needs and are very accommodating to request. I would recommend this place again and again. It's the staff that makes it awesome and they have some compassionate and understanding employees. Thank you!

    My grandfather is a resident at Signature Healthcare, and he loves living there. He has been there for around two years and I can't say enough about his care.The staff is absolutely amazing, they go out of their way to make this difficult time easier on my grandfather as well as our family. During this time where visits are not allowed, the staff has assisted my grandfather in learning how to use facebook, and goggle duo so he can visit and see us. The director of nursing Sherley is wonderful. I have never seen a director of nursing that was so involved with every aspect of patients care. While visiting at the window one afternoon I observed her assisting my grandfather to the bathroom. This is not the first facility my grandfather has been in, so I know that the director of nursing isn't usually helping someone to the bathroom. When my daughter was born, it was Sherley that took the phone and assisted my grandfather so he could see his great granddaughter for the first time. The love and care that the staff have given my grandfather is priceless. I could not ask for a better place for him to be. I want to thank the entire staff for the wonderful care they give and for making it easy for me to leave knowing he is in good hands.

    I have had a personal experience having my Aunt at Charlotte Bay, the staff was always caring and available. If I ever had questions or concerns the staff addressed them promptly and was dedicated to getting me answers. My Aunt was always clean and well taken care of and enjoyed the diversity and freshness of the meals. Overall a wonderful experience for my Aunt and our family. Thank you to all the staff from housekeeping, dietary workers and all of the nurses and CNAs and the wonderful attentive managers.

    I was discharge from Bayfront Hospital, my friend recommended Signature Health Care. I was surprised with the staff, they were attentive and always with a smile. The facility was clean and smelled fresh. Therapy was intense and through. Their Chaplain Dennis is attentive, courteous and encouraging. He made me feel at easy & safe, someone that listen, very accommodating with my family and pet. Nurses and CNA's are professional.

    The MOST TERRIBLE EXPERIENCE EVER! Charlotte Bay Rehabilitation & Care Center IS NOT the place to care for your loved one. Do NOT consider this center. THIS PLACE NEEDS TO BE ACCOUNTABLE FOR NEGLECT & ABUSE TO THEIR PATIENTS!!! Our uncle not only was left in dirty diapers overnight; his room was dirty, his food was inadequate and cold, and even his very basic needs were IGNORED. Our uncle became weaker and more sick at this center and NOTHING was done until his family had to step into this situation. Because our uncle had virtually NO CARE, his family had to move our weak & sick uncle out of Charlotte Bay Rehabilitation & Care Center and into another facility. Thank God his family checked on him very often through the days he was there and saw the neglect & abuse for themselves. They make the intelligent decision to move him. My advice to everyone is: Do NOT consider this place as a possibility for your business. It is both a crime & a sin to not respect and care for these patients.

    If you’re wanting to take your family or loved one to a facility that cares about their patients, this isn’t the place. My UNCLE laid in his urine for hours. Has been neglected for weeks and hasn’t received the rehab needed to return home effectively. Unclean. Uncaring. RUDE. Let’s anyone into their facility after hours. This facility should be shut down and the meals in the dietary department wouldn’t fill the belly’s of a toddler. DOH shut them down or fix the problem. My uncle is NOT alone.
