I have a sister who was recently admitted into this facility, "Avante at Lake Worth" 3 days ago. She is a "Trek" patient, and have special needs and is someone who needs to be watched often. The nurse "Silvana" who I met that night she was admitted assured me that my sister's medications needed to be inputted into the system. She lied in my face about inputting them into the system. They are not trustworthy. My sister had to be rushed to the hospital the very next day due to not having any of her medications. The entire time my sister was admitted there with NO medications giving to her. Very sad and the whole staffing team that night communicated to me that she was in good hands, and there was no need to worry. Wow! My sister seized all through the night going into the next day from the facility not having any of her medications. Poor management. I'm not a person to exaggerate about things, but this first time experience my mom and I had was "Terrible".

This is the worst place I have ever seen. My husband was taken to this rehabilitation center and they almost killed my husband. The nursing staff never contacted me about my husband’s care. They took him off his medication in April and never put him back on it. My husband’s blood is out of control since taking him from this facility. He was abused my the staff. He was denied food. They didn’t do any physical therapy with him. He just laud there and suffered. my husband had neck and back surgery and those people were supposed to do physical therapy and occupational therapy and all they did was take the money from the insurance. He spent eight months in this facility and they almost killed him. What kind of doctor tell a patient when your blood sugar get to 300 then we can give you medication. I will not stop until this facility is closed down. I will find someone who will listen to me about this facility. The staff is incompetent.

My mother has gone here TWICE for respite visits and my biggest problem with Avante is no one answers the phone up front. I call and call and it takes a good 3 hours for someone to answer the phone. Another huge issue is the front receptionist does not take messages to my mother nor to her nurses. And much worse than them not answering the front phones, try speaking to a nurse!! That rings a few times and hangs up on you!! How can a facility get away with not speaking to a family member after they have called repeatedly???? I have yet to ever speak to anyone that cares for my mom while she’s there, only the front desk when she decides to answer it. There is more excuses than answers… And the only “excuse” I get is they are short-staffed, interesting enough if you go to their job website, they are not hiring!! So the 2.8 ⭐️ are quite accurate-

I want to take a minute to recognize an employee who goes above and beyond at Avante. Ms Lola at the front desk is ALWAYS ready to take a few extra minutes when I am trying to advocate for my father from out of state. After months of her helping with tasks from insurance documents, to contacting the various departments; today she went even farther to take his cell phone to him after he was admitted to the hospital. I so appreciate knowing that she is so compassionate to the residents!

I cannot comprehend how a business that is supposed to be dedicated to caring for the elderly cannot even perform as a business with regard to communication. Even this minor detail is severely lacking. You cannot get in touch with your loved ones at any time. The phones are always inoperable. There is no way of reaching the nurses station. You have to repeat yourself multiple times to get anything done. Appointments are not kept with patients who need to go outside the facility.The only time that communication seems to be done is when they're looking to obtain money or avoid being reported.Unfortunately this is actually a better facility than the one my mother was at before this one. Most of the staff is generally nice. However there is no cohesiveness or organization to patient care. It does not seem that one hand knows what the other one is doing. An extremely frustrating experience.

My mom started staying here about 4 weeks ago and the entire time she has been here she has been treated so badly. She was constantly neglected by the nurses and was treated with such disrespect and rudeness. Any time she would press the nurse button they would take hours to come or sometimes not show up at all. Some have even refused to give her things out of pettiness. She is paralyzed and has severe wounds that the nurses were supposed to take care of and since they didn’t she is now septic with really bad infections and had to be rushed to the emergency room today and is now really sick. Even the EMTs who came to take her said the place was horrible and how the nurses are not doing their job at all. This place is also so unsanitary and just absolutely disgusting. Every time I have been in there the whole place smells so bad and so strong of urine. One time I actually saw urine in one of the hallways. Trust me and all of the other bad reviews when we say never to come here. No one should have to live this life. This place needs to get shut down the quality of care is horrible and the department of health seriously needs to come.

9/4/22 - The people that work here are useless, we call them everyday, and never get an answer. I will continue to update my review until someone answers my phone call. 9/5/22 - still haven’t heard from anyone about my father in law. 9/6/22 - still haven’t heard from anyone about my father in law. 9/7/22 - still haven’t heard from anyone about my father in law. 9/8/22 - still haven’t heard from anyone about my father in law. 9/9/22 - still haven’t heard from anyone about my father in law. 9/10/22 - Covid update, got an automated phone call about two people at the facility that have covid, but still no calls about my father in law.. 9/11/22 - no updates, nurses station hangs up. 9/12/22 - no updates, nurses station hangs up. 9/13/22 - nurses station hangs up. 9/14/22 - nurses station hangs up. 9/15/22 - nurses station hangs up. 9/16/22 - nurses station hangs up. 9/17/22 - nurses station hangs up. 9/18/22 - nurses station hangs up. 9/19/22 - nurses station hangs up. 9/20/22 - nurses station hangs up. 9/21/22 - nurses station hangs up. 9/22/22 - nurses station hangs up.

02-01-2023 This has to be the worst place in the world! They have no in house supplies for the residents. They do not encourage your elderly parents or grandparents to keep going. They believe in leaving them in the bed all day and will not feed them or lotion them if they are not able to. THE NURSES are impatient. If you leave your parents here, you truly must not love them. 0/5 ⭐

I can't tell you how pleased I was with the staff at Avante Lake Worth. My dear friend suffered a massive stroke and spent almost two months there. Due to COVID, we were unable to visit and had to rely on the staff for information about him and his progress. I was so happy with both! The staff were always willing to help in any way they could - Ronnie in Admissions would track down someone to give me an update or go visit my friend - the social workers were great - even moving my friend to a room with a window so we could see him from the outside. The physical and respiratory therapists were awesome - they made a ton of progress with him that we never even expected. The level of caring and compassion from the staff there was amazing. If you have a loved one who needs specialized care, send them there - you will not be disappointed.

My mother in law was a patient at Avante at Lake Worth and was recently discharged home. She was not able to care for herself due to a massive stroke and Avante rehab and nursing staff helped her become stronger and back to herself again. The dedication and commitment of this team has been so amazing. She even enjoyed the food, which being a healthcare worker, I know is always a major complaint in facilities. I highly recommend this facility if you would like your loved one to get the best rehab and nursing care. They truly care about their patients :)

My husband was a patient at Avante in Lake Worth from July 2020 til September 2020. His stay was a long and a recuperating one, but the staff was very good to him. The only problem was we had most of the nurses who were so very kind to him but there are always some who are not. A big shout out to Ashley and Ronnie in Admissions who were so very helpful in evey sort of way. And of course Barbara and her staff from therapy for helping my husband out as they did . Avante LW nursing home & rehab saved My life 5 times and cured Me of the Pandemic Covit 19/Corona Virous. In One Month. And another time when i had a kidney Infection. ETS. ETS ETS. A TOTAL OF 5 TIMES THEY HAVE SAVED MY LIFE. I LOVE THE ADMINISTRATION TEAM. THE ADMINISTRATOR NASHICA. ALL THE STAFF, ALL THE PATIENTS THE RESIDENTS. I LOVE EVERYONE.

I’m not one to dog on places, but this Avante facility nearly killed me. I was moved here after a severe accident as this was the only place at the time with an available bed. Twice I ended up in septic shock thanks to lack of care.

AVANTE AT LAKE WORTH STARVED MY PARENT TO DEATH. I HAD TO FIND MY PARENT DEAD, AND THE STAFF DID NOT KNOW. NO ONE SHOULD GO TO VISIT THEIR FATHER ONLY TO FIND HIM DEAD, DEEP IN RIGOR MORTIS (STIFF AS A BOARD).

If you are looking for a good Skilled Nursing Facility for your love ones Avante is the place to be. They have a phenomenal rehab team overall staff is very welcoming and helpful. I would like to thank all of the staff who took care of my Grandfather Terius Dorvil before he passed I truly appreciate all of you guardian angels.

Currently I have a lifelong friend in this facility. This person is begging for a transfer. This person is constantly soiled, has bed sores and is receiving little to NO physician care. This person was brought here for acute rehab and has received ZERO CARE. This person is now bed ridden and going downhill rapidly. I have filed a complaint with ACHA and I’m going to also call news channel 8 They have screwed with the wrong group of ppl. Just know …. I’m coming for this facility

Terrible place. Don't put your loved one there. They steal your clothes. They don't use wipes to clean your family, they use towels. They are thirsty.
