    This place was once a great place, but now has gone down hill fast!! Last night 8.8.21 while I visited my mother, who is there for Short Term PT, I felt as if i was in a horror movie. Bells going off. Patients wandering the halls looking for the nurse or Aide. Make this make sense. 1 RN and 1 Aide to 22 patients. It's a good thing I was there to help my mother to the bathroom. Another gentleman was looking for the nurse who was attending to an emergency on the 2nd floor. It was madness. They need to get their act together. They are mo longer the standard in my book. Places that were below them are now above the. Yes it looks fabulous from the outside, but it sucks internally.

    I wish I could give this disaster zero stars. I brought my Dad here after a stay in the hospital for some rehab on his knee. When we arrived the woman at the desk said she'd alert her supervisor and someone would be out to meet us. An orderly arrived with a wheelchair and escorted us to Room 113. As we walked down the hall we noticed a strong odor. The entire place looked run-down and I was already getting a bad feeling. The orderly left us in the room and said someone would be down in a while. After roughly 20 minutes my sister walked down to the nurses station and asked when someone would come down and speak with us. A very nice nurse showed up a few minutes later to explain that it was her first day and she would be leaving soon. She said someone would be down later to check us in. She said it was shift change so everyone was leaving. A few moments later another worker came in to tell us she worked at the pool but she didn't really work at the facility, she was a "contract" employee and she was also leaving. While we continued to wait for someone to speak with us I spotted a huge cockroach on the bathroom floor. My sister went back to the desk and no one was there. She found the new nurse and told her. She apologized, came down, caught the roach and flushed it down the toilet that my Dad was supposed to use. At this point we had been sitting in the room for close to an hour, no one of any importance had come to see us, and if there's one roach, there's 1,000. I walked down to the desk, where I found four employees standing around talking. I walked behind the desk to retrieve a wheelchair. No one said a word to me as I did this. I brought the chair down to my Dad, helped him into the wheelchair and wheeled him down the hall, through the lobby and to my car. Not one person asked me what I was doing, offered assistance, or tried to stop me. I could have been anyone removing a patient and they didn't say a word! Needless to say, I would never recommend this place and I can't believe they haven't been investigated.

    DO NOT TAKE YOUR LOVED ONE HERE. Please read if you're thinking of sending your loved one here for care. If I could give this place zero stars I could. My dad was transferred here after having hip surgery to receive PT care. The reg flags began from the very beginning when it took the EMTs at least 30 mins to find a supervisor to give us a room assignment. That night I expressed to a nurse that my dad should NOT be given a certain medication because it causes his mental state to worsen. She wrote it down and confirmed she would not give it to him. I thought he was in good hands and comfortable, but I was mistaken. The next morning when I came to visit him, he was in pain, agitated, and trying to explain to me what has transpired that night. He said he had fallen again. NOT A SINGLE NURSE called to explain what had happened or gave context to me the next morning when I arrived. (still have not gotten any information after requesting it, they said they would call me back) I finally spoke to the doctor, who was the only helpful person in the whole building, and he confirmed that they gave him that medication I specifically said not to that night. Not only did they flat out not listen to me, he was under zero supervision the whole night! THEN, they proceeded to do a full cognitive and OT work-up when he's still coming off of the medication (clearly not his normal) AND COULD POSSIBLY HAVE A BROKEN HIP. They didn't do even an x-ray until the next day after they already tried to get him standing/walking and even one of the nurses forcefully rolled him over on his injured side. He was in excruciating pain, it was terrible to watch. Now he's back in the hospital and has to undergo another risky surgery. PLEASE DO NOT TAKE YOUR LOVED ONE HERE. No one cares, no one listens, and the care and love for elderly people just slips through the cracks. So disappointed. Thinking he would be in good hands and get the help he deserves only to find out they made matters 10 times worse.

    If I could put NO STARS, I would! This place is just overall terrible. The food they provide to their patients is almost non-existent. My grandfather being a patient there was terrifying. He wasn't given the proper care he deserves. The nurses panicking when his blood sugar dropped. They did not know how to react in this situation. My grandfather is diabetic and they were giving him very small portions and no snack in between meals. This facility is very unorganized and DO NOT provide quality care. I would not recommend sending ANYONE there.

    A lot of promises and a lot of turnover. Some staff members are awesome but the administration is not responsive whatsoever. If you want to reach nursing or to get an update from staff, this type communication is non existent. It’s a a little better than most assisted living facilities but not by much.

    Our mother has been here since July and we have been so pleased with every aspect of this facility! The staff is so professional and kind. The facility always clean and clutter free. The Recreation director takes time to bring her to activities, the receptionist so friendly. So happy with everything at the Reservoir. Thank you to all

    My mother was recently at the Reservoir for a sixteen day stay. The level of care from the moment you walked through the door from the receptionist, Cindy to the Administrative department, Carol Anne and Becky to the high quality of nursing care. This was a hotel like facility, so clean and the food was delicious.

    The therapists are some of the nicest and professional people I've ever had the opportunity to work with. The kitchen staff is useless. Haven't gotten one meal right in three weeks. The nurses are great except for one who thinks he's the best around. He can't even tuck his shirt in.

    Since i arrive here at THE MANSION that is how i see it all was ok the mansion is a place you want to spend the time when you are ill and you need someone to look after you that when you need to talk someone is there to listen this place is beautiful inside out but THE LADY ADMINISTRATOR WHO RUNS MAKESIT WORTH WHILE she is a very companionable person who listen to you and stand by you my children chose this place and they all live in the STATE OF MASS,,THIS IS MY HOME,,,

    I want to let you know what a fantastic employee you have in Nancy Lewis. She jumped in to help our family with an issue in the facility that she was temporarily helping, not her facility. She made an effort to get involved, when she didn’t need to, because she wanted to make things right for us and also cares about making system wide improvements for all patients. She gave me her cell phone number so that I could personally contact her as the issue got resolved. Your organization is very lucky to have someone like Nancy representing it!,

    I would have given it five stars but I was discharged too soon before I was able to meet basic standards as far as self care and walking, according to Medicare. I filed an Medicare appeal and just got notice that I won the appeal and should be allowed to return. Before I "got notice" that I had won, the Facility said I've have to go to a Hospital ER by Ambulance, to get checked out first, at my expense, and pain on a scale of 8-9 out of 10❗️ It's also Sunday, and no way to get a hold of management to discuss my readmission today. I live alone and can briefly stand in place with severe pain. A letter was sent from Medicare to the facility as well as me letting them know the decision. However there's no management there to talk to and the nursing supervisor doesn't answer her page. Needless to say I'm very upset with no way to cook or care for myself.

    It was the worst nurses/ aids were all extremely rude! My mom was only there for a couple weeks, thank God.. food was awful just everything about the place. When I was taking my mom out with a broken hip I asked the nurse to help me put her in my car, she said NO. Worst experience ever!! Good riddens

    My mother received exquisite care in her two months at the Reservoir. Her team of therapists were compassionate and very knowledgeable. Her nurses and all the staff were amazing!!

    Do NOT SEND YOUR LOVED ONE HERE. I would give it a Zero star rating. They neglected, verbally & physically abused my mother. They laughed and ridiculed her. They made false accusations and lied about treatment. The APRN in charge lied about test results telling us they were negative, when they DIDNT even complete the test. The nurses lied about results and medications. I already reported this facility to the state and some of their employees. It is DISGUST a place like this even exist and let alone in this area. They should be ASHAMED they have doctors and nurses working here. PT/OT and 2 CNAS only good ones. Can’t wait for the state to look at this facility closer now! Don’t send your family here EVER!!!!

    This is NOT a good short term facility. Majority of the nurses lie, ignore and neglect the patients. Do NOT send anyone here. They have been reported to the state. One aide put her hands on a patient, nurses laughed and ignore call bells and giving medications for hours. APRN said tests and exams were completed yet the nurses never ordered it and the APRN lied and gave false results. Video proof. This facility is horrendous and should not be allowed to take care of the elderly. The food was old and burnt. And the nurses sit at the nursing station, eating, laughing and neglecting patients.

    The nurses here are nice and trying their best but the management aka Genesis Healthcare must be terrible. This is the second Rehabilitation Facility I have been to in CT that makes your skin crawl walking into the place. They didn't have socks that even fit on my mom's feet, the bed was broken when we came in, and my mom couldn't use the bathroom because it wasn't properly equipped with those toilet risers. The hospital had reserved a bed for my mother days in advance. The TV's don't even have a clear picture. It's like they are using TV antennas from the 60's. This is not a staffing issue, it's the ownerships lack of funding. Today every hospital has flat screen tv's with the option of logging into your Netflix or Prime acct. This place barely gets cellphone reception. It's pathetic to see how our society treats the elderly. I urge you to provide proper treatment to the patients at not only The Reservoir but all of your long term care facilities. Makes you wonder what the owner is doing with the funding they receive??? Look at actual photos inside the Reservoir, stains on the curtain and old furniture. Does this look sanitary? It's not surprising that COVID spreads so quickly inside these places. 🀒

    My husband did his knee replacement rehab here and now my mother is in there long term care/ hospice unit. Couldn’t ask for a better place! Room’s and staff are fabulous, and it is always immaculate. So pleased !
