    HEALTH ADVISORY WARNING! Do not let your family stay here. My father was here a month and just had to be readmitted to Kaiser ER as nobody has a clue to what is going on and his health was seriously deteriorating. During his stay his condition worsened and both my wife and I are witnesses to there neglect and malpractice. Pills left on the ground, nurse who was caring for him didn't know what his condition was on 2 visits, extremely scary as you should not touch a patient if you don't know the chart. While I waited in the parking lot in the red zone due to staff taking all the stalls I noticed a patient was alone behind facility in motorized chair feeding/petting stray cats near there dumpster area. Then after goes back into building. All is now contaminated. Worse of all the nurse practitioner who had to pull me in a room to speak in private so all clients/family in the lobby would not hear the truth about what is going on there. Outcome of this tragedy is that they blame Kaiser for their wrongdoing and neglect. Once the ambulance came to pick him up and take him back to Kaiser they gave update to the ER department stating that this facility had no knowledge of today's activities or why his condition has worsened. Very sad and I will be taking legal action to prevent anyone else having to come close to dying here due to them not doing there job... God bless and protect yourself and your loved ones. I'm sure they'll have a class action lawsuit within the next few months as I see many unhappy faces...

    The staff may be nice to your face but as soon as they have a chance to leave a patient and do nothing, they will. Other reviews agree even stating that they don't even care about the patients there. Now they are trying to boost their reviews by giving staff rewards for leaving good reviews.

    I wish there was a zero star option. Cockroach infestation, filthy floors in the rooms. Nursing staff does not communicate between shifts, rehabilitation therapy is a joke. My dad is a veteran, sent there to rehab for a partial foot amputation. Itโ€™s disgusting how they have mistreated him. Filing with ombudsman and removing him. How much can I post here? No rehab, no help to the toilet, response time from when he rings for help is about 20 minutes. A patient actually called 911 for help today.

    My stay at Sacramento Post Acute was a remarkable positive experience! As a person who resists change and not looking forward to staying at this place turned out to being one of the most positive experiences ever! After experiencing the love and the tender care that everyone from the administration to the maintenance crew provided me and continues to provide all of its residents is amazing! The dining room also serves as an activities room and has itโ€™s own coffee bar. Thank you Stephanie and Sabina for spreading the love. Thank you to Sam, Stephanie, Adriana, Katie , Gwen, Maria, for motivating me, pushing me and inspiring me to move on to the next chapter. The people in Occupational Therapy and Physical Therapy are awesome! I love this place and I am grateful that I was able to continue with my rehabilitation here!

    I certainly would not recommend this place. My mother was admitted to this place after being transferred from another facility in Northern California during Covid. He condition declined weekly as the staff just kept her medicated and did not actually attempt any physical therapy. The place constantly smelled unsanitary and looked dingy. Don't find out how I did.. if you are here looking for reviews please schedule a visit and see for yourself before admitting a loved one...

    My grandpa suffered stroke last year October and was transferred from UcDavis to Vibra in Folsom where he showed some improvements. Unfortunately, he needed to be transferred to a stepdown care and I chose Sacramento Post Acute due to its higher rating than other facilities but i was wrong. He was transferred to this facility at early December 2021 and I thought he's in great hands. Dec 15th 2021 was first time I visited him (needed appointment to visit due to COVID which I understand), I was so disappointed. The staff walking me to where my grandpa was couldn't find his room. We went from one room to another looking for him. During the next visits, I observed my grandpa's condition regressed. At one point, I tried talking to the RN in charge of his care but I was told by another nurse sitting at the station that "she left early" so I cannot talk to anyone about him. On December 29, 2021, when I visited my grandpa, I saw him very weak, showing respiratory distress and hypothermia. That was the time when I lost my hope that he will recover. He was sent to Mercy ER at that night and placed on DNR. I felt that all the efforts helping him to recover from stroke at UCDavis and Vibra were in vain because of this facility. From Mercy Hospital, I decided to bring him home until he passed. I belive this place is so understaffed. I hope they will stop taking new patients if they can't take care of them properly and safely. It's not the fault of Healthcare providers working here but the administration running this facility.

    I had full knee replacement surgery on 4/1/22 and was transferred to Sacramento Post Acute the following evening. There was a major problem with my pain medications because under state law, providers of SNF/rehab facilities are limited to prescribing nothing stronger than 10mg Percocet. I received 1mg IV Dilaudid every 4 hours in the hospital but when I arrived at Sacramento Post Acute, they couldnโ€™t give me anything stronger than 10mg Percocet, which for me, a chronic pain patient with a high opiate tolerance, is nothing. I choose to return to the Sutter Hospital ER where they got my pain under control and sent me back to Sacramento Post Acute with a prescription for 4mg oral Dilaudid. None of this was the fault of Sacramento Post Acute. My pain specialist should have waited until I knew which SNF/rehab facility I was going to and should have sent my pain medication order directly to the contract pharmacy for Sacramento Post Acute. Instead he sent my prescription (everything is transmitted electronically now) to my local CVS and told me to pick it up and bring it with me to the rehab facility, also telling me NOT to open the bag. SNF/rehab facilities cannot accept external prescriptions. All of this is for any patients who need more than 10mg Percocet every 4 to 6 hours. The nursing staff, CNAs, PT and OT were fantastic, the entire staff was kind, considerate and as helpful and accommodating. They went above and beyond the call of duty. Iโ€™m having my left knee replaced as soon as the right knee is healed and I will be returning to Sacramento Post Acute after the hospital. My only mild criticism is that it can sometimes take 45 minutes to 1 hour for the staff to respond to my call light, but if I made them aware that it was urgent, I always got a prompt response. Iโ€™ve NEVER tested positive for COVID-19 but, because Iโ€™m not vaccinated, I was quarantined in a private room with my own bathroom for my entire stay. I highly recommend Sacramento Post Acute and Iโ€™m NOT a staff member nor do I have any vested interest in this or any of the associated facilities. Michael D Bailey Sacramento, CA

    Since i have been here i have high expentation. Julie Swift has made it happen if i can give her 15 stars i would but the system only allowed me to give her 5. I like her efficiency of work and i would recommend her and this place to anyone. She has a passion in what she does. I am very thankful for everything that is being done as i continue to recover.

    I continue to be impressed with this facility. I worked there for 8 + years and I miss the staff and residents. The administrator there is top notch and the quality of care of the residents is the best I have seen in the area.

    This facility is outstanding. My daughter was fortunate to be able to be at Sacramento Post Acute. The Cna's, nurses, front desk, therapists, etc. Are all top quality. I was pleased with the cleanliness. I was a pesty visitor and they all made me feel at home. I could go on and on about this facility. Corie enjoyed her stay. Thanks to all of you.โ™ฅ๏ธ

    My medical care was excellent but did not liked to move to different rooms. Some of the rooms were run down with damage furniture and two of the beds I was in had damaged controls. Difficulty sleeping due to noise in hallways.

    The quality of care is ok. Most are overworked. You can wait up to 2 hours for your light to be answered. The nurses assistance are wonderful. The food is pure garbage. Of course they said I could Door Dashed it. At my cost of course. Example: 1 pancake, 1 piece of sausage and maybe 1 cup of oatmeal. Drinks include 4 oz of milk and Kool Aid with water for every meal. It is truly disgusting. Don't come here.

    Total hip surgery, couldn't praise the PT&OT therapies enough.The overall experience was very good,My wife had a hard time parking but the meat loaf was better here than hers so I will overlook the parking this time.

    My brother is being cared for at Sacramento Post Acute . He has been at a few other care centers and none of them were as clean, and professionally run. I was greeted and given clear information when I arrived to see my brother. I truly thanked the Lord for placing him in such a welcoming place. He was started on rehabilitation immediately hurray,

    My fist time visiting, Sac Post Accute facility. As a Hospice Aide, was very happy with the staff. Everyone was very friendly, CNA was amazing. The only downside is, trying to find a parking space.

    Service providers have been wonderful. Response to needs in the rooms of the patients can be pretty slow but in fairness, they are short staffed with CNAโ€™s.

    Sacramento Post-Acute is such an amazing facility with great people. My aunt was a resident there and she was given the best care towards her wounds , she was able to heal and come back home! Thank you all.

    This place is one of the best Iโ€™ve been.All nurses seemed to be knowledgeable. Care about your health, even the ones I didnโ€™t care for once you get to know them they all do care. Iโ€™ve went to three the only one I gained weight. Thanks everyone

    I transfered to this rehab after having a heart attack and a stroke, which I could not move my whole left side, and my face was drooping and could not talk,, And Sacramento Post Acute did extensive rehab on me, the facility has great people and they really care about the residents. Thanks to the people in this facility, I am walking, talking driving and living a normal life.
