    My sister was checked into this place right before the July 4th holiday. During that time we had several family meetings outlining our expectations and what the center needed to release her to home care once again. During that time, her brand NEW wheelchair was stolen. We advised the staff in a family meeting (nurse, rehab specialist, therapist and another person who may have been a business manager) that the chair she was currently using was not her chair, described the make of the chair which is clearly printed on the back and requested it be found and returned. She is now home and I have attached pictures of the chair they gave her. In addition, her nurse said if her original chair was stolen, it needed to be replaced by the center. She had them waiting to take her home - but the chair she got was old and broken. Both of the arms are broken. What is she supposed to do with this? IF you send your loved one here, be prepared to visit several times per week. Especially in the beginning. That is the only way to ensure they do their job SOME of the TIME.

    If you love your loved ones DO NOT send them here. Quality of care is terrible! Nurses/Assistants act like they have better things to do. No one is around to help with needed. They will turn the call light off and never actually fix the issue. Have to ask for things that are supposed to be supplied or done automatically. Very lack luster place. I will be speaking to the Administrator as soon as possible. This place is not worth the money. Send your loved ones somewhere that will actually care for them!

    This place is a nightmare that you don’t want to experience. Nurses and most attendants do not assist patients when help is needed. The facility is old and poorly kept with no physical therapist on staff to assist patients. Food is cold and poorly prepared. With COVID they don’t sanitize properly and do not serve sealed individualize drinks. Instead they pour cheap drinks from a big jug into old cup for people to drink. Bathrooms are old, dirty with inadequate hot water for bathing. They had a 2.0 rating which was more than they exhibited. I don’t know who lied and said a 3.4 rating, this place should be shut down. Being in jail could not be worst. You really put your life on the line coming into this β€œHELL HOLE”‼️

    My second time inAutumn Homes. The food was marginal but Shelly the kitchen manager, was very helpful in grabbing something for me to eat. Even if it wasn’t on the menu. The staff were accommodating and helpful and friendly. I enjoyed my time there. The maintenance on the building can use some work. Thanks.

    I recently visited my brother at Long Green and I was very pleased with his care and rehab. I was also extremely impressed with Mrs. Galloway,MSW. She is truly dedicated in making Long Green a positive experience for all residents involved. Thank you Mrs. Galloway and God Bless you and all of the staff and residents at Long Green.     My experience was great! The staff was pleasant. The Social Service Director, Mrs Galloway was helpful, informative and caring when we had to meet. Overall, I would highly recommend this facility for your loved ones. Mrs. Galloway goes above and beyond to ensure that the residents are treated with care and respect. My loved one is now a resident of Autumn Lake Long Green. I can Sleep with peace every night knowing she’s in good hands. ❀️

    Staff provides good service. Matt and Heather on the rehab team are really good at their jobs. Nursing stays on top of you. I really like the activities/recreation with Connie. She keeps it fun. I have also learned a lot in the Wellness Program. Mrs. Traci taught me a lot about addiction and is caring and supportive. Especially when you are going through changes. She teaches how to maintain your sobriety and how God helps us .

    Thank you for my stay. The PT staff were stupendous. The regular staff Cynthia, Nicole, Frank; nurses Louis, Jonathan were Γ lways on top of every thing and took great care of me. The only negative was the food. What facility has good food?LOL THANKS TO ALL of the people there. Cynthia Levario

    The care at Autumn Lake at Long Green is beautiful. They take care of my uncle as if there own. They always keep me updated when change happens. I have recommended and will continue to recommended Autumn Lake at Long Green to others.

    I came with the Evangelism team from Mount Pleasant Church and Ministries. We had a great time. Thanks for allowing us to come in and fellowship with the residents there. I've noticed each time we come there are never any women in our services. Where are your women residents? We want to share Christ with them also πŸ₯°

    Exceptional Administrative Staff. Particularly in the Social Services who works with other systems to get the patients needs met before discharge and The Admission Department which is very friendly and understanding. The staff communicates the patients needs on a daily basis and are friendly. As a Public Health Case Management Professional, I highly recommend THIS Autumn Lake over the other facilities

    The staff is the most polite and cooperative staff I've ever met. The food is good. My questions were answered promptly. The therapy department was great, especially Gabby. I would recommend this place for anyone who needs time in a nursing home.

    The staff was welcoming and the facility was clean. My grandmother was well kept and resting peacefully in her room. One of the aides came in to see if my grandmother needed anything and she also asked if I needed anything which I thought was very nice. Thanks, Autumn Lake at Long Green for taking care of my grandmother during her respite care!

    A friend is at autumn lake healthcare at Longview. She is receiving daily wound care and some physical therapy. My first impression was positive since there was not an overwhelming smell of urine when I walked in the door. Yes, there are smells as you enter, but it seems it is addressed, not ignored. The caregivers seem competent, many are friendly and go the extra mile to help when possible. All the people I spoke to were approachable. The facility is an aging, but charming building. Along with the standard wings of patient rooms, there is a social area, sun room, overgrown courtyards, and spacious cafeteria. There are activities throughout the week for various interest including movie night with popcorn, Bible study group, and bingo. The director and social worker and other staff I spoke with seem committed to patient well being. Renovations are planned to improve the building. My friend is sometimes frustrated when staff can’t meet her requests or someone is short in answering her. She is allowed to have outside food, even though she feels blamed for pre-existing fruit flies when staff tells her to cover her food well. There is a small space for patient food in the refrigerator, but it can only stay a few days. Overall, I am impressed with the care my friend receives, especially as she is high need, being mostly bed bound. Most importantly, her deep and extensive wound is healing under their careful practices.
