I'm hoping the arizona department of health services shuts down this disgusting disgraceful business. Idk how they are able to pass health codes with such low standards.

Alot of those problems that other people put down aren't all there I think cause there are apps to block a call and such but.. you guys have horrible customer service aren't organized communicating wise or file. and takes a very long time to connect to anyone there. Sorry you guys have the same treatment of a greyhound station.

The man at the front desk is the absolute WORST human I’ve ever encountered. Talk about unprofessional, mean, and demeaning. I would highly recommend having someone friendly being the first person people encounter at a business. After I left, other professionals were apologizing for how he treated and talked to me. So embarrassing and so unprofessional. I will say, Bernardo is special licensing is absolutely WONDERFUL! He helped me out despite the man at the front desk being absolutely awful towards me.

After filing a complaint at AZDH about my daughters preschool due to several students reporting that a teacher had used tape on another child’s mouth to discipline them, ADHS did not contact me or my child to discuss the complaint. Not only did they fail to contact us for an interview they listed on their findings that they had discussed this with the complainant which is a flat out lie. I called and complained that no one had contacted me but nothing was done.

You guys can always contact governor ducey office for assistance in getting your manners handle when this state agency isn't doing much of anything to help. Not sure if senate does anything but you can contact Senate sinema office they are usually good at responding

My wife and I just had our first dose of the vaccine on Friday. The entire process was well managed and well executed. Every person we interacted with at the Cardinals stadium location was professional, courteous and very efficient. Thanks to all the great folks associated with the vaccine deployment here in Arizona!!!

These clowns declined me for medicaid when I fall way below the poverty line. I hope they as taxpayers enjoy fronting my hospital bills whenever I get a mild ailment. Which are 10x what they would be if I were insured. I can't pay them so lol.

I had a mix-up with my address for my speech pathology assistant license and I spoke with Monica this morning. She was SO quick to help, accommodated, answered my questions, and made my day with her kindness. I love living in AZ and having such friendly and hospitable people helping problem solve. Made appointment for my parents for State Farm Stadium vaccine on 1st day the appointment system was open for 1B. Not simple but with persistence I was able to schedule. The people at the stadium were fantastic. Everyone was positive, patient and helpful. 1st and 2nd vaccine took less than 30 minutes from arrival to departure(including 15 minute wait after shot). There was a sign on 2/11 that they have given out more than 1,000,000 vaccines at the stadium. Thank you and keep up the good work.

The staff was courteous and very helpful, even 15 minutes before closing on a Friday (Whew! Barely made it.) I was able to park right outside the entrance at a metered space. There was only one person ahead of me, so I was in and right back out within a few short minutes.

The online Covid sign up was a frustrating and a disaster, having to pick one randomly potential appointment time after another, with all selections being rejected, then eventually giving up and calling our doctor's office who was able to get a time slot for my 90 year old mother. The online program should have shown the available times by date and allowed the user to pick one. However, upon arriving at the State Farm Stadium, the medical staff and their process of administering the vaccine was great.

Just trying to retrieve childhood immunization records. Follow instructions and get an "error, try again" message. Try again ten more times to no avail. Call the office for support and get a recording that says to fill out the form on the internet. Rinse, Repeat

The office of DHS in regards to EMCT and EMT-P certification should be investigated for intimidation, abuse of power, reactive rather then proactive mindset and investigative ethics needs to be evaluated, They have made it evident they are out to collect money from us they give no help to any of the good folks they “certify”
