    This place is absolutely horrible! The way they take care of the patients is unbelievable. The place needs to be tore down immediately. They donโ€™t clean ever and always say they do, the fixtures are ancient. I would not put a dog in this place. Iโ€™m trying to find a facility to bring my mother to until I am able to care for her at home. Her personal hygiene items that I provide are always missing. And the excuses are endless. Her clothes are always missing itโ€™s just ridiculous.

    My mother in-law was in this place for 3 weeks. I canโ€™t believe this facility only has one cordless phone for around 40+ patients. Everyday we tried to call her, we were constantly put on hold for over a half an hour or more just to talk to a nurse and this was not the weekend! The care plan meeting was a joke! The person that conducts these meetings didnโ€™t even show up to the scheduled meeting. During the call it was clear that the employees were incompetent to our case. Today we picked her up to bring her back home and the trash bags that were given to us reeked of urine. Everything that we brought her was thrown in a clear trash bag with snacks that we brought her. By the way, we were told that they have laundry services! This is totally Unacceptable! I would never recommend this facility to anyone!

    The facility is poorly maintained..equipment is probably 50 years old and doesn't appear very clean. They haven't put money into their infrastructure since it was likely built. Sad bc we paid 5000 for a 7 day stay and when we were discharged the items he needed for home weren't even delivered yet and he was told his bed had been taken by someone else so they couldn't hold him until the am...I think that speaks for itself....I'll drop the mic on that one..not need to go into basic hygiene and the fact that his clothes that we brought his clothes on the first day he arrived and they weren't given to him until his last day there. Very disapointing.. some staff were very kind and helpful..which made it tolerable.

    It has taken me a week to post a review. My husband was at Community Care Center for 2 weeks during that time his clothes were stolen or went missing his watch was broken and thrown on the sink. The place was dirty. The staff did their best but the place was under staffed. You can see especially on the weekends. I wish I did not have my husband there. He is now in the hospital again

    My father was in this facility and he was told to pee and poop in his pants while sitting in his wheelchair because they didn't have the staff to take him to the bathroom. He was in such bad condition when we transferred him to the Hospital the nurses were appalled and said they would have reported us for abuse if we were responsible for his care prior to bringing him in. I was given a phone number to call to report them for abuse by the charge nurse.

    My dad didn't get his clothes until the day he was discharged. We paid out of pocket for a 1 week stay. The hope was that he'd receive intense 3 times a day therapy bc he wanted to get home after raising my mom from covid and him being in the hospital for over a month with covid. He just wanted to get home to bury her. We handed in his clothes on day 1 and he never wore them until discharge. We still have not received our refund that we were told was processed at the end of last year. They kept talking about care months down the road. Really feel like they didn't start much rehab bc they figured he'd be there for a while. The case worker didn't have a bed, walker , bedside commode, wheelchair ready for him until a day after he was already home. The coordination of the ambulance ride etc. Was all us. I feel like he wasn't a priority...Tallk about a stressor for someone who couldn't even stand and who is over 6'3. When we asked if he could just stay until things were ready at home. He was told his bed was already filled. Finances were not discussed just drop a 5000 check by. We still have not received a run down of services provided and expenses. They don't invest anything in the facility. It's pretty gross and run-down. I am a nurse and their beds have to be 30 years old. We were atleast pleased with the pleasant receptionist and a few staff members.

    Would never recommend this place for anyones loves one to stay! Unprofessional, disorganized, and their lack of care for their patients/residents truly shows with the โ€œcareโ€ they provide. Donโ€™t let what they allow you to see fool you, they are definitely not a fan place for anyones loved ones.

    My dad was housed at this facility for some time and though I do believe there are some professional staff who take their job seriously not all are up to par. My dad had dementia and he wouldn't eat on his own, but according to some of the staff on a certain shift he would eat at every meal time. But that was a lie because he wouldn't swallow his food, we attempted to feed him day after day and he would just hold it in his mouth. He went without eating for who knows how many days before he was transferred to South Florida where he was given a feeding tube. He lost so much weight in the months before his passing

    So glad I stopped in to tour l! Very clean, smells great! The staff is so kind and welcoming. It feels like family! As they say, Donโ€™t judge a book by its cover! Therapy was busy and patients stopped me to say how good the therapy department is. Each staff member I passed in the hall looked up, smiled and said hello.

    This is for the therapy team!! The COTA Destiny is super great!! She takes good care of my granny!!! Iโ€™ve seen much improvement in my grandma with her care!!

    My mom. was here for rehab. Needed more therapy. But they went about it like the insurance would not pay any more . Social worker went about it totally wrong. You can file an appeal to have the therapy extended. The food was horrible. I wouldn't give that food to an animal. My cat ate better than that.

    My grandmotherโ€™s leg was allegedly broken during in room therapy. They allegedly denied it. I am under the impression that my mother insisted upon X-rays, which confirmed a broken leg. My grandmother allegedly also fell out of her wheelchair.

    Absolutely a disgrace. This place should litterly be shut down. Where do I start. The building is is horrible need of repairs. Mold on the ceiling (I have pictures. Look at the rotting deck by the front door. Smell the urine in the hallways. The critically understaffed. Several of the classic are rude and hateful to the patients. Litterly waiting an hour to go to the bathroom. My God! How about a house keeper (kelly) stole a ring off my mom's finger telling her she was going to clean it. We filed a police report. She was fired. Cna dropout my mom not helping her scrapped her back I have pictures. The food served it not fit for a dog. Si much more

    They don't take care of the patients especially night shift they drug them up so they won't bug the staff my mother almost died in there don't let your loved ones go there she went in good talking ,alert,but in few days in there she didn't even recognize her own family that went every day to see her they say they would put oxygen took 3 and a half days that day we took her out we do much better at talking care of her at home

    I not like this place i did for frist for my mom now nope in year 2020 the virus broke out my mom was suffering from liver damage form one drug long ago one of the employees work there gave some the virus that includes my mom she frought twice she dye in the hospital in Tampa some gave to her twice i lost my best friend an sister i cared about then in 2021 my dad end up there after he couldn't walk great he had type 2 diebeaties an need meds what happened easy when his sugar drop they try to help they didn't do it correctly by call 911 to save him it was too late they call me a hour yes s hour after he dye tell me i had no Dad that hurt more i now have no parents cause of this place

    My husband was here for 6 days mid June 2023 recovering from back surgery. Was treated very roughly by cna. The rooms are small, dark, and the bathroom is small, does not have handicap height toilets. Also two rooms share one bathroom. Food sucked. The ONLY positive was the physical therapy staff. They treated him with respect. Will never recommend this place to anyone.

    My mom was talking doing ok on Thursday and Friday they transferred her to the hospital. She passed away that night. Would not suggest taking your loved ones here

    Great facility. The staff are kind and very attentive. My husband was there for a few months and I was very happy with his care and how they attended to all our needs.
