We were in town for a baseball tournament and this restaurant was suggested by some friends. I will never come here again. In my salad, which I took a bite of, was a used band aid. This thing was in my mouth. It was the most absolutely disgusting thing that I have ever experienced at a restaurant. The server showed very little empathy toward the fact that I just chewed on someones used band aid. Apparently there was no manager on duty - he was in St George. Don’t go here. Nasty. Uncaring. Filthy. I can’t even think about the situation without gagging. Absolutely horrible.

Picture this: it's prom night in Vernal. We walk up to the restaurant, and there is a couple dressed for the dance in front of us. While standing there waiting, 8 more couples come up. "Oh, we're all together," they say. The wait went from 2-3 minutes to twenty in five seconds flat. We got our table, and ordered. The pizza is excellent, and despite being short-staffed with a new cook on the line, it was pretty quick. We were feeling good. Then Lisa, our server (she might be the manager? I don't know) told us she appreciated our patience and gave us a German chocolate cake on the house. We didn't ask for it, she just did it. I heard someone else ask for some, and their server told them they had run out, so we might have gotten the last piece. It was delicious. In short, the service was exemplary, the food was great, if I lived closer, I would go here frequently. (Lisa was not wearing a visible name tag, but that was the name on the ticket, so I hope she was the one who got our tip.)

We came here for dinner, and were blown away. My wife and I haven't had pizza this good since we visited Italy, and I never expected to find such great pizza, or the mind-blowing fresh burrata, in Vernal. Everything looked great and the staff were super helpful, and food came out pretty quick and very hot and delicious. I got the pistachio pizza, which I enjoyed, and my wife asked for a bite and ended up eating a whole piece it was so yummy. The burrata was a great starter as well, creamy and delicious. Overall a must eat in Vernal!

They were really busy the night we stopped by but the staff was still very friendly and gave us great pizza recommendations. We were pretty hungry so I didn’t get a pic of the full pizza 😳. This is what’s left of a Pistachio pizza and Tri-Fungi pizza? I would highly recommend but get a reservation if you have a group on a weekend. Great food, service, price and local beer.

We had a Pizza and it was deliciosso! The young waiter did a great job, his only downfall came when he asked if we wanted "any Dessert" and we said "no, unless you have a Cannoli", which he replied "what's a Cannoli", for that response I'll have to change my rating for this fantastic Italian restaurant to a 4.999999.😁 "youngsters, right", to the owners, buy the kid a Cannoli

The pictures had us sold! It LOOKS like delicious authentic wood fired oven pizza. It is in fact, super basic pizza masquerading as italian cuisine. This pizza would have tasted better if I picked up a jar of marinara at Smith's and had them use it. I am a marinara sauce fanatic. Theirs was bad. After one disappointing bite of the pizza I went straight for the crust hoping it was as good as it looked... more disappointment. Adding insult to injury, it was very expensive, almost $100 for 2 adults and 2 children under 5. The service was average, we waited for a long time at each phase of the dining. We had just entered when the host returned to the front desk. He answered the phone without greeting us and proceeded to take a to-go order in front of us. I suppose he hadn't been trained to use "hold" on the phone. After we were seated we weren't waited on for at least 10 min. I can forgive slow service on a Friday night, or some unmet expectations, but not spending top dollar on below average pizza. I'm certainly never coming here again. I'm giving it 2 stars only because the ice cream was good, and I got to watch the second half of the Jazz game there.

In a word, amazing! Do yourself a favor and eat here if you are ever in Vernal. I've never had a gluten-free pizza this good. Not only was the staff welcoming but the pizza is crazy good.

Our very large mountain bike team had a family pre-race dinner the night before a race in Vernal two weeks ago. Antica Forma catered pasta, chicken, meatballs, salad and bread for 300 people and did a fabulous job. Everyone raved about how good the food was! They were great about providing gluten-free pasta as well for our team members with allergies. Highly recommend!

Awesome food! Awesome service! Great portions! If I could give more than 5 stars I would. Food came hot and quickly and everything was absolutely delicious! We will definitely be back. Worth stopping here if you ever get the chance!

Some of the best Italian food I've had in Utah! Make sure you love garlic though as I do! The complimentary garlic bread was amazing!!

One of the best pizzas in town, hands down. We had the Antica Forma gluten free pizza which was phenomenal, would definitely recommend and we will be coming back in the future ourselves. Love the wood fire oven as well, imparts a really nice smoky flavor as well.

Might have been the best pizza I ever had. I sat and watched her make my Pesto Pistachio Pizza toss it in the wood fired stove and deliver it to my plate! Absolutely delicious!

Wow! This restaurant is far above average and has some of the finest foods we have tried in a while. Their garlic bread was the absolute best we have ever had, and that was served gratis before our meal. The actual entrees were complex and unique. We will recommend our friends and family come to Vernal just to try this restaurant. The fresh peach pie for dessert was perfection. We fed our family of five (three young children) for about $80.

I had an excellent experience at Antica Forma with my colleagues. Our server was kind and had great recommendations. I had the La Nonna pizza (mozzarella, prosciutto, arugula, and parmesan reggiano) and the bruschetta. Both were delicious and Antica Forma definitely has the best pizza in Vernal.

Pizza was good as was appetizer. The dough was very good. The 2 little bread sticks they brought out at first were amazing but very small portions. The service outstanding. My only issue is that it is pretty pricy for what it is.

We came in for lunch around 2:30 and there only seemed to be one waiter which worried us. However, he did a good job and the service was still great. Food was amazing and the pizza tasted really wonderfully made. I’d definitely go again and would recommend it.

I am not a fan of specialty pizza. But this place was great. The pistachio pizza was SO good. My wife ordered it because I always take the safe bet (meat lovers in this case). My pizza was good for sure but the pistachio was AMAZING. The first time we went we each ordered a pizza and it was a lot of food. The next time we split a pizza and it was closer to what we could eat. We have also taken our kids there. They ate off of the kids menu and enjoyed it.

Food was super tasty everything we ordered tasted fresh and just like we ordered it. We got seated pretty quickly but, after being seated service was not terribly attentive. Restaurant was not very busy but, still felt like we got a bit ignored. Seated right by the kitchen so we got to watch the pizzas being made which was great for the kids and family. Food is great. If your in a hurry you may need to order ahead. If your taking it slow wonderful place to stop in.

I’d like to consider myself someone who makes good pizza. Eating here humbled me. The crust was perfect, with good fermentation, you could tell the dough wasn’t made the same day. Toppings were simple and just as perfect. Great atmosphere for casual dining, brick oven right up front so we could see the pizzas being made. Highly recommend.

Amazing flavored pizza. Ordered the Il Tartufo that had a crispy thin crust that I could eat all day. Came with Garlic bread that was also great. Slower service, but well worth the wait. Will be back again to try their other pizza flavors.

Delicious pizza and cooked very quickly. My kid ate it no problem and he only ever eats pepperoni or cheese. Cooked perfectly. We tried the pistachio. I never would have put that on a pizza but it was my absolute favorite there.

Disappointing. I was here for dinner last September, and the service was excellent. Sadly the food left a lot to be desired. I ordered the mushroom calzone and I was served raw, folded over pizza dough with nothing inside, with some cheese, sauce, and mushrooms on top. The cheesecake was good though. I was told “the cook was new.” I wasn’t even mad, just disappointed. Hope the cook learned how to cook.

Really good pizza and pasta. The only complaint was that my wife's salad had ingredients that she said were not on the menu, and she would not have ordered it if she had known. I guess of you like a little added flair to your order thats a good thing, (I do and enjoyed the salad). Overall I don't know how my experience could've been better. My wife would probably give it a 4.

Wow! Don’t walk, run. Genius Italian pizza. The pistachio!!’ I’ve had Pizza in Naples. I’ve had it in New York. I’ve had it everywhere and there’s nothing that tops Antica Forma. Sadly the Tiramisu is more cream than fingers and not as amazing as the pizza. Skip the gelato. Yes I know you don’t agree but again, if you’ve had real good gelato you’ll know, this is not it. Go for the pizza, and the amazing staff.
