Upon arrival, I was asked to fill out 4 different paper forms, even though I've been there in the past...

👍👍 To say Thank you, is an understatement. This office, especially the young doctor who cared for me and saved my life. I am alive today because of him. Thinking I just had a chest cold and needed meds. He looked past that and said I have a potential blood clot. He sent me directly to the hospital....
