    To make a long story long...My sister is currently a patient in this facility. This is her 3rd stay. The first time she was there her care and progress were remarkable. She made progress and was discharged home. Within a week she was back in the hospital and was discharged back to Dignity. The first few days she did well and was making progress. After a few days she started to display confusion and agitation and even delirium. I spoke to the nurses and they immediately went in and began to ask her questions. Her birthdate, her name, where she was and standard questions. She answered them correctly and they informed me that she seemed fine. I insisted that she was not. A member of the staff heard the conversation and later came to me and noted that they noticed that she was acting confused and "not herself". She also noted that she had regressed significantly over the last two days. She was no longer able to stand up independently, walk or take steps and required assistance to get on and off the toilet. This was later confirmed by her PT and OT. The following day they asked me to come for "family training" as they were preparing to release her to go home the next day. When I arrived for my appointment they were helping her get from the bed into a chair. This task required 3 staff members. When I expressed concern that she was not ready to be released and that her family would not be able to provide her with the level of care and support she required they insisted that they could provide me with effective training to care for her at home. They insisted that they could teach me in 30 minutes how to provide the level of care that 3 trained staff struggled to provide. I asked to speak to her caseworker Byron. He also insisted that it would be possible. When I said that her home was not set up to meet her needs and that we could not provide the level of care she would need he asked my sister if she felt we could. She replied that she believed we could. I expressed to her care team that based on her current mental state she was unable to determine the scope of care she would require. Once again they insisted that she was lucid and understood the gravity of the situation. When I asked for a list of resources for additional support Byron said he would get back to me. He also said he would follow up with her Radiologist and Oncologist. He never did. When I talked to the nurses and staff about my concerns they were very dismissive. I left the facility very unhappy. The day she was set to be discharged we insisted that she be taken by ambulance to the hospital. The hospital determined that she was indeed experiencing confusion and cognitive concerns. After a few days in the hospital at her request she was released back to Dignity. She has been there for several days and is still unable to stand, walk, or perform any selfcare. They are currently providing her some memory retention therapies and basic PT exercises that she performs while sitting. She has experienced no progress yet they plan to release her to go home. She is not able to use the toilet, bedside commode or shower. Her big accomplishment is sitting in a wheelchair for a few hours each day. On three occasions this week she has called me and said that she needed something and that nobody was responding. Last night I had to call the nurses station and ask them to bring her water. The facility is very clean and the rooms are spacious and private. The CNA's are friendly and seem genuinely concerned about the patients and provide quality care. If the care team had better listening skills and communicated with the patients medical team and care providers it would lead to better outcomes for the patient. They focus solely on the task of rehabilitation while ignoring pertinent medical conditions. When pushed they say they are a rehabilitation facility not a hospital or acute care facility. Her daughter has POA and they must be reminded that they should contact her regarding my sister care and progress. I wrote a goal on her board. It's really basic and what we were promised.

    I have been to rehabs a few times the past year. Dignity is by far the best. Didn’t know about it until Kim came to my hospital room and told me about it. My experience was wonderful from beginning to end. Loved the private room and bathroom. The staff was kind and gentle. The food was actually good and very personalized. If you go try the turkey burger…. Yummy!!! To my surprise when I was released a couple of the upper management team was waiting at the front door clapping and thanking me!!! Very sweet 😊 Also special thanks to Kim and her continued help, director of case management and that department. Thanks to all the physical and occupational therapists. Thanks to all the CNAs and RNs and everyone else who made my stay as pleasant as possible.

    My mother stayed at this facility and I am so thankful to RN, CN & also all the staff here who have made my mom's recovery process easier for her. We cannot thank enough the OT(specially Monica) and PT teams for their patience, readiness to help, and motivational words. We are grateful they shared some easy tips and tricks to combat bodily pain from surgery. Special thanks to Franz, who knows exactly when to push during exercises. I have not seen a facility before with such down to earth people, who are so kind and treat everyone with respect no matter what their background is! The doctors are also very helpful during their visits. The house keeping crew is awesome too! The room set up is exceptionally good with built in bathroom which can provide privacy to every patient with their individual needs! FIVE STARS! We feel blessed to have such a wonderful experience. Thank you all.🙏

    I Raymond Martinez had a wonderful time at Dignity Health Rehabilitation hospital because the staff was GREAT just to name a few Kameron, Steven, Aaron, Scott, Chris and everyone else cause they have me doing everything for myself such as getting from my wheelchair to my bed or to take a shower or to the bathroom and so on, Thanks again for them that I leave here with a big smile on my face... Thanks again...

    I've been out of the rehab hospital for a little over a month now. I have to express my gratitude to this facility for my amazing experience. The staff in particular made this traumatic experience so much more bearable. My physical therapist Doug and my occupational therapist Katie spent every opportunity they had to help me improve. They hyper focused on problem areas and I improved almost daily bristles of the motivation and support they provided. They would give me the credit but i can't thank them or the facility enough. Amazing people doing amazing work with a real heart and passion for recovery. Thank you. Special thank you to some of the nursing staff as well, Grace, MK❤️, all the cna's all so attentive and caring.

    My first time with a stroke and I hope that no one has to go through that experience. I was sent to a hospital then to Dignity Rehab. The Hospital was a disaster it was dirty and staff was there for three hours then left no matter what you may need help with. I was hoping that all medical facilities were not the same. After I arrived at Dignity I got the same treatment as the hospital and I thought not again with the bad service. However I soon learned that they give the same great service all the time 24 hours a day. The doctor even had a great sense of humor and kept me up to date with the medicine and treatments that I had to do everyday. The Nurses I had were very understanding and paid close attention to what I was going through and feeling. They let the doctor and other staff know what is going on so that I had a great place to get better. I am one the people that heals quickly and expressed the thoughts that I needed to go home since I felt caged up and could not get the full exercise I thought I needed. the PT Trainers listened and also came to the same conclusion and released me with record time. Now that I am home I have made remarkable recovery so far. When I left I was using a walker to get around and one week later I am not using any thing to walk and not dragging my foot and walking up and down stairs without assistance. If they would not have listened to me so that I could recover as soon as I did the process may have taken a lot longer to get to where I am now. I still have a long way to go but without Dignity Rehab I don't think I would have been on the correct path to recovery. Thanks to everyone that works there for the help that I received. From management to all the nurses therapist and everyone else for such a great experience. Highly recommend them to everyone that needs rehab. Update to this Post DO NOT USE THIS FACILITY Before I went to this rehab I made sure that they were in my network and all of their staff agreed that they were in my network and that everything would be covered under my insurance. Since 3 months after leaving I am billed with unbelievable debt from this institute and they are refusing to help with any of the bill. Just as a reminder they will stab you in the back with a huge bill that you can not afford and then threaten you to collect it. Just be aware that of the billing problem with these people. My druthers use anyone but this place.

    I forced my wife to be discharged earlier than she was supposed to. The facility is great , PT and OTs are excellent , but all the speaking Tagalog caretakers are bad. It's a nightmare to my wife. She cried after I took her home. She got hurt more in there than when we first moved in. Please don't let your loved ones suffer with these unprofessional folks.

    My grandfather suffered a stroke and has a long list of previous medical conditions that are very complex. In the past, We have gone through a lot of medical facilities, rehabs, and hospitals. As we were very biased negatively by past experiences in life, we came in expecting low standards of treatment for him..... But this facility has restored our faith in the ability of medical staff to treat patients with the utmost compasion and care. Absolutely every single medical employee we have encountered has been nothing short of amazing. They throughly care for every single patient and it shows in the way they treat, care, and talk to the patient and also the family members. My family and I would like to recognize the staff members who have taken amazing care of my grandfather and by extention us even with a language barrier as the language my Grandfather and most of my family speaks is spanish. My grandfather has stayed on the 2nd Floor currently with these amazing staff: RN, Grace - Rehab Asst, Lisete - PT, Lesley - OT, Michael - SLP, May - CNA, George - RN Krilleza. Sorry if I mispelled any of your names. And my apologies if I forgot any. This team of medical employees are the best! So experienced and knowledgeable. Every single question we had for them, they took the time to answer until we fully understood. Id highly recommend this facility to anyone needing it! You are in good hands!

    Where not ready to discharge her at stranded time. Rushed to get paperwork ready. Most meds they showed they sent in where never sent. Followed up but they would not send them in even though it was on discharge paperwork. Couldn’t answer questions we had at discharge. Shameful and they don’t care . Avoid going there!

    Amazing facility. Exceptionally caring nurses and ancillary staff. Administration is on point with regard to running a highly functional rehabilitation facility understand the needs of patients and families. They really do care about all aspects of patients care and focus on returning someone to prior level of function. The nurses are extremely responsive and are very compassionate. I highly recommend.

    Nursing staff and rehab assistants were all excellent. However the reason I gave 4 stars is there were a couple of rehab assistants that were neglectful in their care for me. I had a little fall while in the bathroom when they left me unattended. Rehab therapists were all spectacular and they made it fun and interesting. Dr.Gao is excellent.

    The facility was very clean,, the nurses really nice and the PTs were very professional. They made sure my husband got the full amount of exercise he needed.

    I had the best work out to recover from a surgery that went wrong. Amy ways positive and showed such a good time. I give her 5 stars she’s he best.

    My husband suffered a stroke while we were celebrating my birthday in Las Vegas. He spent 3 weeks at Dignity Health Rehabilitation Hospital. The staff from check-in to discharge were amazing. What stands out is the true professionalism and dedication. Thank you to all!

    My stay was very productive and very beneficial to my recovery. All the staff was very helpful and friendly. Special thanks to nurse Irene, therapists Tina, Jennifer, Franz and PJ. MJ was great on the ad min end.👍

    I've been here 3 times and the staff/team has always been good and strong. Even if I had Kardashian money - I would still wait for a spot here. The food is excellent. They will rebuild you up to health. And since they have outpatient now, they will keep you up to health.

    If I had the $, this hospital would have law suits . I guess I'll have to.wait til they're finished killing my mother then use the money from her home to sue them . My mother's entire family tell these people that my mother is having huge cognitive issues but since she can tell you her birthday and name they insist she's ok to make decisions but since she's been there she's gained 80lbs and my mother now cannot get up at all anymore . Do not send your loved ones to this rehab

    They've been absolutely hell to deal with . I have power of attorney and they never return my calls ever and lie to your face about everything.Byron has no business being in healthcare ...car sales more like . Patronizing and unintelligent. Avoid this place

    Kim is very helpful and very knowledgeable of the facility. She was aware of my concerns for my mom’s physical challenges as an amputee. The staff CNA’s, RN’s, OT’s, PT’s, ST’s, doctors and including housekeeping and front desk were amazing, took care of my mom treated her with respect. I do recommend Dignity Rehab hands down over anywhere else. But the only down part is the case manager Byron, promised things but doesn’t follow through I hope they get someone else that follows through with supplies to care for your love ones at home and constant communication with the family and patients.

    My mother's case manager Tamika, as well as the entire staff, did a wonderful job helping my mom transition back towards independent living. Their kindness, compassion, and professionalism shined through the actions of every staff member. They have my undying gratitude.

    My mother had several surgeries in her lifetime and she’s been in different rehab facilities, so far Dignity Health Rehabilitation Hospital is the best rehab facility she’s ever been to. The professionalism of the healthcare workers in this facility is outstanding. They definitely patiently understand the care of every patients need. Thank you Kim for reaching out and keeping us informed. This is my mother’s second time in this facility and their patient care remains outstanding. To all the therapists, nurses, doctors housekeeping THANK YOU for all your hard work.
