To
make
a
long
story
long...My
sister
is
currently
a
patient
in
this
facility.
This
is
her
3rd
stay.
The
first
time
she
was
there
her
care
and
progress
were
remarkable.
She
made
progress
and
was
discharged
home.
Within
a
week
she
was
back
in
the
hospital
and
was
discharged
back
to
Dignity.
The
first
few
days
she
did
well
and
was
making
progress.
After
a
few
days
she
started
to
display
confusion
and
agitation
and
even
delirium.
I
spoke
to
the
nurses
and
they
immediately
went
in
and
began
to
ask
her
questions.
Her
birthdate,
her
name,
where
she
was
and
standard
questions.
She
answered
them
correctly
and
they
informed
me
that
she
seemed
fine.
I
insisted
that
she
was
not.
A
member
of
the
staff
heard
the
conversation
and
later
came
to
me
and
noted
that
they
noticed
that
she
was
acting
confused
and
"not
herself".
She
also
noted
that
she
had
regressed
significantly
over
the
last
two
days.
She
was
no
longer
able
to
stand
up
independently,
walk
or
take
steps
and
required
assistance
to
get
on
and
off
the
toilet.
This
was
later
confirmed
by
her
PT
and
OT.
The
following
day
they
asked
me
to
come
for
"family
training"
as
they
were
preparing
to
release
her
to
go
home
the
next
day.
When
I
arrived
for
my
appointment
they
were
helping
her
get
from
the
bed
into
a
chair.
This
task
required
3
staff
members.
When
I
expressed
concern
that
she
was
not
ready
to
be
released
and
that
her
family
would
not
be
able
to
provide
her
with
the
level
of
care
and
support
she
required
they
insisted
that
they
could
provide
me
with
effective
training
to
care
for
her
at
home.
They
insisted
that
they
could
teach
me
in
30
minutes
how
to
provide
the
level
of
care
that
3
trained
staff
struggled
to
provide.
I
asked
to
speak
to
her
caseworker
Byron.
He
also
insisted
that
it
would
be
possible.
When
I
said
that
her
home
was
not
set
up
to
meet
her
needs
and
that
we
could
not
provide
the
level
of
care
she
would
need
he
asked
my
sister
if
she
felt
we
could.
She
replied
that
she
believed
we
could.
I
expressed
to
her
care
team
that
based
on
her
current
mental
state
she
was
unable
to
determine
the
scope
of
care
she
would
require.
Once
again
they
insisted
that
she
was
lucid
and
understood
the
gravity
of
the
situation.
When
I
asked
for
a
list
of
resources
for
additional
support
Byron
said
he
would
get
back
to
me.
He
also
said
he
would
follow
up
with
her
Radiologist
and
Oncologist.
He
never
did.
When
I
talked
to
the
nurses
and
staff
about
my
concerns
they
were
very
dismissive.
I
left
the
facility
very
unhappy.
The
day
she
was
set
to
be
discharged
we
insisted
that
she
be
taken
by
ambulance
to
the
hospital.
The
hospital
determined
that
she
was
indeed
experiencing
confusion
and
cognitive
concerns.
After
a
few
days
in
the
hospital
at
her
request
she
was
released
back
to
Dignity.
She
has
been
there
for
several
days
and
is
still
unable
to
stand,
walk,
or
perform
any
selfcare.
They
are
currently
providing
her
some
memory
retention
therapies
and
basic
PT
exercises
that
she
performs
while
sitting.
She
has
experienced
no
progress
yet
they
plan
to
release
her
to
go
home.
She
is
not
able
to
use
the
toilet,
bedside
commode
or
shower.
Her
big
accomplishment
is
sitting
in
a
wheelchair
for
a
few
hours
each
day.
On
three
occasions
this
week
she
has
called
me
and
said
that
she
needed
something
and
that
nobody
was
responding.
Last
night
I
had
to
call
the
nurses
station
and
ask
them
to
bring
her
water.
The
facility
is
very
clean
and
the
rooms
are
spacious
and
private.
The
CNA's
are
friendly
and
seem
genuinely
concerned
about
the
patients
and
provide
quality
care.
If
the
care
team
had
better
listening
skills
and
communicated
with
the
patients
medical
team
and
care
providers
it
would
lead
to
better
outcomes
for
the
patient.
They
focus
solely
on
the
task
of
rehabilitation
while
ignoring
pertinent
medical
conditions.
When
pushed
they
say
they
are
a
rehabilitation
facility
not
a
hospital
or
acute
care
facility.
Her
daughter
has
POA
and
they
must
be
reminded
that
they
should
contact
her
regarding
my
sister
care
and
progress.
I
wrote
a
goal
on
her
board.
It's
really
basic
and
what
we
were
promised.
